    Intervention outcome and mechanism of epimedium total flavonoids on aged rat hippocampus during inflammaging progress based on gene expression profile

    【Abstract】 ObjectiveTo investigate the mechanisms of inflammatory cytokines network in the development of inflammaging and EF intervention outcome and its mechanism.MethodsHealthy male Sprague Dawley rats were divided into 3 groups: 4 m young group with 4 months rats, 24 m aged group with 24 months rats treated with distilled water intragastric administration and 24 m+EF group with 24 months rats treated with EF by intragastric administration for 3 months from 21 months, 10 each group. The gene expression profiles of rat hippocampal tissues from rats in every group were analyzed by using inflammatory cytokines and receptors cDNA microarray,respectively.The differential expressed genes were
    screened.At the same time, the levels of inflammagingrelated inflammatory cytokines proteins expressions of TNFα,IL6 and IL10 in rat hippocampal tissues were evaluated by enzyme linked immunosorbent assay. ResultsIn rat hippocampal tissues, compared with those in 4 m group, there were 9 differentially upregulated expressed proinflammarory genes and  6 downregulated expressed antiinflammatory genes in 24 m group. Compared with those in 24 m group, there were 8 upregulated expressed antiinflammatory genes and 11 downregulated expressed proinflammatory genes in 24 m+EF group. Compared with those in 4 m group, the levels of TNFα and IL6 in the supernatant from rat hippocampal tissues in 24 m group increased significantly(P<001). As compared with those in 24 m group, the levels of TNFα and IL6 in the supernatant from rat hippocampal tissues in 24 m+EF group decreased significantly (P<001). But the level of IL10 in EF+24 m group  was significantly higher than that in 24 m group (P<001). The ratio of TNFα to IL10 and IL6 to IL10 in 24 m group increased significantly as compared with those in 4 m group(P<001), and decreased significantly in 24 m+EF group as compared with those in 24 m group(P<001). ConclusionsThere is upregulation of mRNA and protein expressions of proinflammatory cytokines and receptors in the aged rat hippocampus, which may eventually contribute to disequilibrium of the network of pro and antiinflammatory cytokines and receptors and disequilibrium of response of pro and antiinflammatory system and high inflammatory state, which provides new mechanistic insight into inflammaging. EF may interfere in inflammaging via rebuilding the new equilibrium of the network of pro and antiinflammatory cytokines and receptors and equilibrium of response of pro and antiinflammatory system, which might be caused by downregulation of expressions of proinflammatory cytokines and receptors and upregulation of expressions of antiinflammatory cytokines and receptors.

    【Abstract】 ObjectiveTo investigate the relation of anxiety and depression with social support among elderly patients with hypertension.Methods500 elderly patients with hypertension were investigated by using simple coping mode questionnaire,selfrating anxiety scale,selfrating depression scale and social support rating scale.ResultsThe total score of selfrating anxiety scale had negative correlation with the total score of social support rating scale,objective social support,subjective social support and utilization of support in social support in elderly patients with hypertension in community,respectively(r=-0113,r=-0136,r=-0142,r=-0102);The total score of selfrating depression scale had negative correlation with the total score of social support rating scale,objective social support,subjective social support and utilization of support in social support in patients with hypertension in community,respectively(r=-0132,r=-0142,r=-0156,r=-0127).ConclusionsAnxiety and depression correlate with social support respectively among elderly patients with hypertension in community. Enhancing social support among elderly patients with hypertension in community maybe contribute to their physical and mental health.

    Effects of memantine on serum SOD and NSE in patients with Alzheimers disease

    ObjectiveTo observe the effects of memantine on serum SOD and NSE in patients with Alzheimers disease(AD).MethodsSixtyseven elderly patients with AD were randomly divided into memantine treatment group and routine treatment group. Thirtynine healthy subjects were chosen as normal group. The serum concentration of SOD was measured by chemical colorimetry. The serum concentration of NSE was measured by enzymelinked immunosorbent assay (ELISA).ResultsAfter 6 months of treatment, the level of serum SOD and the score of MMSE in memantine treatment group were higher than those in normal treatment group; the level of NSE in memantine treatment group was lower than that in normal treatment group. After 6 months of treatment, the serum level of SOD in memantine treatment group increased, and the serum level of NSE in memantine treatment group decreased; the score of MMSE in memantine treatment group was higher than that before treatment.ConclusionsThe therapy of memantine could reduce oxidative stress and get better efficacy in treating AD.

    ObjectiveTo observe the changes of brain natriuretic peptide (BNP) levels after drugeluting stent (DES) implantation and to explore the effects of BNP levels on cardiovascular events in elderly patients with stable coronary artery disease (CAD).MethodsBetween 2007 and 2009, we conducted a cohort study involving 88 old patients with stable CAD. All were confirmed by coronary angiography (CAG). The data of clinical characteristics, CAG and percutaneous coronary intervention (PCI) were collected. Baseline samples for BNP measurement (microparticle enzyme immunoassay for BNP) were available in total patients. After DES implantation, 29 old patients with stable CAD underwent the measurement of BNP again. The subjects were divided into two groups according to the baseline level of plasma BNP,such cardiovascular events as death, nonfatal ACS, repeated PCI and rehospitalization were recorded.ResultsBNP levels in 29 patients after PCI were significantly decreased(from 23043±4881 to 14933±1749 pg/ml, P=00382). After 125 months followup, higher baseline BNP levels were associated with significantly increased risk of death, nonfatal ACS, repeated PCI and rehospitalization (P<001).ConclusionsThis study confirms that removal of fixed stenosis after PCI decreases the plasma level of BNP. Baseline elevated BNP concentrations are predictive of adverse cardiovascular events by 1 year after DES implantation in old patients with stable CAD.

    ObjectiveTo investigate the complications and longterm clinical outcomes of pacemaker implantation in elderly patients aged more than 80 years.MethodsConsecutive old patients with cardiac pacemaker implantation from December 1,2001 to December 1, 2007 at the Institute of Geriatric Cardiology were selected. Clinical characteristics, complications and longterm outcomes were compared between patients aged ≥80 years (very elderly group;n=60) and patients aged ≥65 years and <80 years(elderly group;n=60).ResultsClinical characteristics were not significantly different between two groups except age(P<005).The incidences of lead perforation and pain complication in the very elderly group were significantly higher than those in the elderly group(P<005).Improvement of symptoms and life quality, defect and complication of pacemaker, lead parameters after 1 year  were not significantly different between two groups(P>005). The incidence of heart failure in the very elderly group was significantly higher than that in the elderly group(P<005).ConclusionsPacemaker therapy in the very elderly patients is safe and effective. More attention should be paid on lead perforation and pain after pacemaker implantation in the very elderly patients.

    ObjectiveTo investigate the effect of HPV infection on immune escape of cervical cancer cells.MethodsThe level of HPV mRNA was explored by realtime PCR, meanwhile, the deposition of C3b was detected by flow cytometry.ResultsThe levels of HPV mRNA and C3b deposition were significantly increased in human cervical cancer tissue, especially in elderly group, which contributed to the migration and intrusion of cervical cancer cells.ConclusionsThe results indicate that the infection of HPV might involve in migration and intrusion of cervical cancer cells through inhibiting the deposition of C3b.

    ObjectiveTo explore the relationship between carotid artery intimamedia thickness(IMT) and cardiovascular risk factors in elderly hypertensive patients.Methods205 elderly hypertensive patients were divided into two groups according to IMT, the IMT thickening group and IMT normal group. Age, course, blood pressure and related biochemical indicators were compared between two groups.ResultsThe IMT thickening group had older age and longer course than IMT normal group. The levels of blood pressure, LDLC, FPG, CRP were significantly higher and the level of HDLC in IMT thickening group was lower than those in IMT normal group.ConclusionsCarotid artery IMT is closely related to the cardiovascular risk factors in elderly hypertensive patients.

    ObjectiveTo observe the clinical effects of “Mengxing decoction” in the treatment of depression after apoplexy in the elderly patients.MethodsEightytwo subjects were randomly divided into treatment group  treated with western medicine and “Mengxing decoction”and control group  treated with western medicine.The clinical effects were observed in the two groups respectively two and eight weeks after treatment.ResultsThe scores of Hamilton depression rating scale and neural function defect scale in the treatment group were superior to those in the control group significantly (P<001).Conclusions“Mengxing decoction” has good effect on poststroke depression.

    ObjectiveTo observe the curative effects of fluvastatin in treatment of mild and moderate Alzheimers disease.Methods120 patients with mild and moderate Alzheimers disease were randomly divided into the control group and the study  group. The control group received conventional therapy,while the study group was administrated orally with 40 mg of fluvastatin every evening additionally. The levels of TC,TG,LDLC,HDLC as well as the scores of MMSE,CDR and ADL were compared between the two groups at week 4 and week 12 after treatment, respectively.ResultsThe improvements of TC,TG,LDLC,HDLC and the scores of MMSE,CDR,QOL and ADL were significantly greater in the study group than those in the control group (P<005).ConclusionsFluvastatin can postpone the progress of mild and moderate Alzheimers disease, as well as lowering serum lipids.

    Effects of AUTOCPAP on blood pressure variability in elderly hypertensive patients with obstructive sleep apnea syndrome

    ObjectiveTo study the effects of autocontinuous positive airways pressure(AUTOCPAP) on blood pressure variability in elderly essential hypertensive(EH) patients with obstructive sleep apnea/hypopnea syndrome(OSAHS).MethodsSixty elderly EH patients with OSAHS were enrolled,according to apnea hypopnea index(AHI) and nocturnal lowest SaO2(SaO2min). Seventeen cases of EH with mild OSAHS (5<AHI≤20,SaO2min 85%~89%) and 43 EH with moderate or severe OSAHS (AHI>20,SaO2min<85%), were given routine antihypertensive and AUTOCPAP treatment for 20 daysAll patients received 24 h blood pressure monitoring before and after treatment. The variations of blood pressure variability(BPV) were recorded. BPV was measured by means of the standard deviation (SD) of systolic and diastolic pressures during 24 hour and circadian:24hSSD,24hDSD,dSSD,dDSD,nSSD and nSSD.ResultsAfter treatment of AUTOCPAP, there were significant difference in the variations of nSSD in EH with mild OSAHS patients (P<005), the variations of 24hSSD,24hDSD,dSSD,dDSD and nDSD were not significant(P>005). After treatment of AUTOCPAP, the variations of 24hDSD,dDSD were not significant(P>005), the variations of 24hSSD, dSSD and nDSD were significant(P<005), the variations of nSSD were more significant(P<001)  in EH with moderate or severe OSAHS patientsConclusionsDuring the routine antihypertensive treatment,AUTOCPAP can significantly modify BPV in elderly EH patients with OSAHS. Compared with EH patients with mild OSAHS,the change is more obvious in moderate or severe OSAHS patients.

    ObjectiveTo observe the outcome of patients with corneal perforation associated with ulceration after amniotic membrane transplantation combined with conjunctiveal flap covering.MethodsTen patients (10 eyes) were treated by amniotic membrane transplantation combined with conjunctiveal flap covering. Continuous use of bandage contact lens and eyedrops were given in the second day postoperatively. All patients were followed up for 312 months.ResultsThe ocular surface became stabilized and healing of corneal perforation was obtained after the operation. Three patients with secondary glaucoma received cyclocryosurgery. During followup,no recurrence of ulceration or severe complication was noted.ConclusionsAmniotic membrane transplantation combined with conjunctiveal flap covering is a safe and effective method for patients with corneal perforation due to ulceration.

    ObjectiveTo investegate the independent pharmacologic effects of albumin and furosemide on hypoproteinemic elderly patients with acute lung injury or acute respiratory distress syndrome.MethodsForty patients with acute lung injury or acute respiratory distress syndrome aged more than 60 were randomly divided into intervention group, receiving treatment of albumin and furosemide, and control group, receiving treatment of furosemide. The oxygenation index, the level of albumin, net fluid loss were measured.ResultsThere were no significant differences in baseline characteristics between two groups. Albumintreated patients had increased oxygenation index (P<001), increased serum albumin (44 g/L vs 095 g/L at day 3, P<001) and more net fluid loss (P<001) throughout the study period. Although the mobidity of albuminfurosemide treated patients was lower, there was no significant difference compared with the control patients. The albuminfurosemide treated patients could live longer than those control patients (P<005).ConclusionsThe combination therapy of albumin and furosemide in hypoproteinemic elderly patients with acute lung injury or acute respiratory distress syndrome could significantly improve oxygenation index, increase negative fluid balance, prolong survival time, with no significant benefit in mortality.

    Correlation of erectile dysfunction with lower urinary tract symptoms in elderly people

    ObjectiveTo investigate the degree of lower urinary tract symptoms (LUTS) in elderly people with erectile dysfunction (ED) and evaluate the correlation between ED and  LUTS.MethodsTotal 67 patients were divided into mild group, moderate group and severe group according to international prostate symptom score (IPSS). Every patient took a 5item version of the international index of erectile function ( IIEF5) test. The peak systolic velocity (PSV) and end diastolic velocity (EDV)of cavernosal artery were measured by color doppler ultrasonography.Then the results were statistically analyzed.ResultsThe means of IIEF5 scores and PSVs decreased along with the progression of LUTS and significant differences were found when mild group comapared with moderate and severe group(P<005).There was no significant difference of EDVs among three groups(P>005).ConclusionsThe sexual life quality in elderly persons with ED decreases along with the progression of LUTS and hemodynamic effect may be the cause.

    Clinical analysis of cardiopulmonary resuscitation in the elderly patients

    ObjectiveTo study the status of 335 patients who showed cardiac arrest and received cardiopulmonary resuscitatian (CPR), and to investigate the clinical characteristics to improve the success rate of CPR.MethodsA restrospective study was carried out in 335 patients who showed cardiac arrest. According to the age, the patients were divided into two groups. The success rates of return of spontaneous circulation (ROSC) and cerebral resuscitation were compared between two groups. The starting time of CPR, the starting time of the establishment of artificial airway, defibrillation time and the dose of adrenalin were analyzed and compared.ResultsThe success rate of ROSC was 2092%, while the cerebral resuscitation rate was only 065% in the elderly group, compared with 3242% and 385% in nonelderly group. There was significant difference in adrenalin dose(P<005), while no significant difference in the starting time of the establishment of artificial ventilation and CPR, CPR lasted time and defibrillation times (P>005) between two groups.ConclusionsThe success rate of CPR is poor especialy in the elderly. The key to improve CPR is to emphasize the underlying disease, strengthing the emergency medical service system.

    ObjectiveTo detect the gene ffh in streptococcus sanguis, and to analyze the homology of ffh between streptococcus sanguis and streptococcus mutans. MethodsStreptococcus sanguis and streptococcus mutans were cultured in anaerobic atmosphere and the genome was obtained by the alkaline method. A pair of primers were designed in the most reserved region of the sequences from GenBank database in order to obtain the unique polymerase chain reaction products.ResultsThe unique product was found and obtained in streptococcus sanguis and streptococcus mutans.ConclusionsThe gene ffh could be acquired from streptococcus sanguis,and the homology of the acidresistant gene ffh in streptococcus sanguis and streptococcus mutans is high.

    ObjectiveTo observe the efficacy and toxic reaction of  pemetrexed or docetaxel as secondline drug for  advanced nonsmall cell lung cancer (NSCLC)in elderly patients.MethodsFiftysix elderly patients with  relapse NSCLC were divided into two groups.In singleagent pemetrexed group,26 cases received pemetrexed at a dose of 500 mg/m2 on first day,and repeated every 3 to 4 weeks. In singleangent docetaxel group,30 cases were treated with docetaxel at a dose of 35 mg/m2 weekly for 4 weeks as a  cycle.ResultsThe effective rate in pemetrexed group and docetaxel group was 192%and 167%respectively;the median survival time was 98 months and 81 months, respectively;1year survival rate was 192% and 200%, respectively.In two groups,  the toxicities were tolerable.There was no grade  Ⅲ or Ⅳ  hematological toxicity in pemetrexed group.ConclusionsElderly patients with relapse NSCLC could benefit from singleagent pemetrexed or docetaxel as secondline treatment. Pemetrexed has a more tolerable toxicity than docetaxel.

    ObjectiveTo investigate the trends and hot points of the researches on geriatrics in Jiangsu province from 1998 to 2007.MethodsThematic analysis was used to investigate the thematic distribution, course distribution and hot points of the researches on geriatrics in Jiangsu province from 1998 to 2007. Results The total number of literatures of ten years of the themes ranking top 20 was 2552, making up 5704% of the total literatures on geriatrics. The timedependent trends of the themes were unclear. The main directions of the researches on geriatrics in clinical medicine were treatment and diagnosis of the diseases, and the main directions in basic medicine were pharmacology and physiology. The hot points of the researches on geriatrics were bone fracture, diabetes mellitus, dementia, coronary heat disease, aging and constipation, which were common diseases in elderly people. Conclusions Jiangsu province has carried out lots of researches on geriatrics, while there are still some problems, such as relative less researches supported by foundations and less theory researches. This study provides reference to the future development of geriatrics in Jiangsu.

    【Abstract】 ObjectiveTo investigate the efficacy of the anticoagulation for acute exacerbation of chronic obstructive pulmonary disease(AECOPD) with coexisting suspected acute pulmonary embolism(PE).MethodsFortytwo inpatients with severe AECOPD with a positive Ddimer assay were divided into group A (a value of PaO2 was normal) and group B(hypoxia). The two groups were randomly redistributed into anticoagulation subgroups (A1 and B1) and nonanticoagulation subgroups (A2 and B2). The consequences of the management were surveyed and assessed.ResultsCompared with  B2 subgroup, there were significant differences in dyspnea, PaO2,Ddimer(P<005) in B1 subgroup.ConclusionsAnticoagulation should immedidately be given to patients with severe AECOPD who have risk factors for venous thromboembolism or high probability for PE.

    ObjectiveTo explore the therapeutic effect of vaginal bridge repair plus pelvic fascia suture in the treatment of vaginal posterior wall defect.MethodsThe data of 83 patients undergoing vaginal bridge repair plus pelvic fascia suture were retrospectively analyzed. The average time of followup after operation was 2242 months. The change of POPQ degree after the operation was analyzed to investigate the recovery of the pelvic anatomy after the operation. The improvement of the defecation functional disorder was estimated by observing the defecation and the PISQ12 sexual life questionnaire was used to evaluate sexual life quality after the operation.ResultsThe average time of the vaginal bridge repair was 15 min. None of the operations had complications. Only one case recurred. Recurrence rate was 12%. 812% of the patients felt the improvement of the defecation functional disorder.2.63% of the patients had algopareunia. The scores of PISQ showed no significant difference between preoperation(7957±656) and postoperation(7862±640).ConclusionsVaginal bridge repair plus pelvic fascia suture have certain therapeutic effect in the near future. It can improve the constipation and has no effect on the sexual life.
