    Epidemiological characteristics of chronic respiratory diseases in the elderly

    Chronic respiratory diseases impose a heavy burden on the elderly, which significantly influence their quality of life. This article reviews the epidemiological characteristics and risk factors of some common chronic respiratory diseases, including chronic airway inflammation, lung cancer, idiopathic pulmonary fibrosis and tuberculosis, aiming to provide insights for clinical prevention and treatment of chronic respiratory diseases in the elderly.

    Epidemiological characteristics of chronic neurological diseases in the elderly

    Neurological diseases have become the leading cause of disability and the second leading cause of death globally. The prevalence of most neurological diseases such as stroke, dementia, and Parkinson’s disease increases sharply with age. A proper understanding of the epidemiological characteristics of common neurological diseases in the elderly can help meet this challenge actively. This article reviews the prevalence, incidence and mortality rates of several chronic neurological diseases commonly seen in elderly patients, such as Parkinson’s disease, Alzheimer’s disease, other forms of dementia, cerebral small vessel disease and sleep disorders.

    The elderly population is at high risk for chronic kidney disease. As the country with the largest elderly population, China faces severe challenges in the prevention and treatment of chronic kidney disease. This article summarizes the current epidemiological characteristics of chronic kidney disease in the elderly in China and primary, secondary and tertiary prevention strategies for chronic kidney disease, aiming to enhance awareness and provide reference for preventing and reducing the incidence and progression of chronic kidney disease in the elderly.

    Correlation between serum creatinine-to-cystatin C ratio and physical performance in elderly people

    Objective To investigate the correlation between serum creatinine-to-cystatin C ratio and physical performance in the community elderly. Methods A total of 203 cases of elderly people who underwent physical examination in the community were enrolled in this cross-sectional study. All the elderly underwent the assessment of short physical performance battery(SPPB) scale, and they were divided into poor physical function group(0-9 points) and good physical function group(10-12 points) based on the SPPB score. The general data were compared between the two groups. Spearman correlation analysis and binary logistic regression analysis were used to analyze the relationship between serum creatinine-to-cystatin C ratio and physical performance. Results The levels of age, 5-time chair stand test, hemoglobin, bilirubin, urea nitrogen and cystatin C were higher, and the levels of grip strength, pace, appendicular skeletal muscle index(ASMI)score, vitamin D, serum creatinine/cystatin C ratio and ifree triiodothyronine were significantly lower in poor physical performance group than those in good physical performance group(all P<0.05). Spearman correlation analysis showed significant relationship of serum creatinine-to-cystatin C ratio with grip strength, pace, ASMI and SPPB score. Binary logistic regression analysis showed that serum creatinine/cystatin C ratio was the independent influencing factor of physical performance. Conclusions Serum creatinine-to-cystatin C ratio is strongly associated with decreased physical performance in the community elderly. It may serve as a simple serological indicator for identifying the decline in physical performance.

    Correlation between preoperative peripheral inflammation phenotypes and postoperative delirium in elderly patients: a latent class analysis based on inflammatory markers

    Objective To evaluate the relationship between preoperative inflammatory status and postoperative delirium(POD) in the elderly patients using latent class analysis based on peripheral inflammatory markers. Methods A total of 240 elderly patients scheduled for unilateral total hip replacement at Affiliated Jinling Hospital, Medical School of Nanjing university were enrolled in this study. Arterial blood samples were collected before anesthesia, and the levels of C-reactive protein(CRP), tumor necrosis factor-alpha(TNF-α), interleukin-1β(IL-1β), IL-8 and IL-6 were detected using enzyme linked immunosorbent assay method. The incidence of POD was assessed using Confusion Assessment Method(CAM) within 3 days after operation, and the patients were divided into POD group and non-POD group. Latent class analysis was used to categorize the patients based on peripheral inflammatory markers. Multivariate logistic regression analysis was used to investigate the independent risk factors for POD, and receiver operating characteristic(ROC) curve was used to analyze the predictive value of hyperinflammatory phenotype for POD in the elderly patients. Results The levels of CRP, TNF-α, IL-1β, and IL-6 showed significant differences between non-POD group and POD group(P<0.05). Latent class analysis using these four inflammatory markers classified the patients into hyperinflammatory or hypoinflammatory phenotypes. Logistic regression analysis showed that hyperinflammatory phenotype(OR=12.15, 95%CI: 5.57-26.5, P<0.001) and age(OR=1.07, 95%CI: 1.01-1.14, P=0.034) were the independent risk factors for POD. ROC curve analysis showed that the AUC of hyperinflammatory phenotype in predicting POD was 0.779(95%CI:0.716-0.847). Conclusions A preoperative high inflammatory state has a good predictive effect on the occurrence of POD in the elderly patients undergoing unilateral total hip arthroplasty.

    Predictive effect of CONUT score on tolerance of postoperative adjuvant chemotherapy and prognosis in elderly patients with stage Ⅱ/Ⅲ colon cancer

    Objective To explore the predictive value of CONUT scoring system in the elderly patients with stage Ⅱ/Ⅲ colon cancer who received postoperative adjuvant chemotherapy. Methods The data of 86 patients who underwent postoperative adjuvant chemotherapy for colon cancer in Nanjing First Hospital from January 2018 to January 2022 were retrospectively analyzed and divided into low CONUT group and high CONUT group according to the preoperative CONUT score. The clinical characteristics, chemotherapy compliance and complications were compared between the two groups, and the survival prognosis was further analyzed. Results The incidence rate of grade Ⅲ/Ⅳ neutrophil deficiency was significantly higher in the high CONUT group than that in the low CONUT group (P<0.05), and the days of chemotherapy delay was significantly longer than that in the low CONUT group (P<0.05). Survival analysis showed that the patients in the high CONUT group had significantly worse disease free survival (DFS) and overall survival (OS) than those in the low CONUT group (P<0.05). Conclusions The CONUT score can be used to assist in evaluating the response to postoperative adjuvant chemotherapy and clinical prognosis in the elderly patients with colon cancer.

    Comparative study on the diagnosing value of ACR-TIRADS and C-TIRADS for thyroid nodules in elderly

    Objective To compare the diagnosing value of American College of Radiology Thyroid Imaging Reporting and Data System(ACR-TIRADS) and Chinese Thyroid Imaging Reporting and Data System(C-TIRADS) for thyroid nodules in the elderly patients. Methods Retrospective analysis was performed.A total of 71 elderly patients(including 97 nodules) with complete thyroid ultrasound images and surgical pathology in Affiliated Hospital of Nanjing University of Chinese Medicine were enrolled in this study. Ultrasound classification and fine needle aspiration(FNA) treatment recommendations were made according to ACR-TIRADS and C-TIRADS, and the diagnostic performance and recommended FNA rate of the two systems were compared. Results The pathological diagnosis as the gold standard,the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of ACR-TIRADS in the diagnosis of thyroid nodules in the elderly were 89.30%, 77.30%, 93.10%, 68.00% and 86.59%, respectively,compared with 89.30%, 68.20%, 90.50%, 65.20% and 84.54% of C-TIRADS(P>0.05). The overall recommended FNA rate, and the acupuncture rates of benign and malignant nodules of C-TIRADS were significantly higher than those of ACR-TIRADS(63.92%, 68.18%, 62.66% vs 21.65%, 22.73%, 21.33%, all P<0.05). Conclusions Both ACR-TIRADS and C-TIRADS have high diagnostic value for thyroid nodules in the elderly, while C-TIRADS has a higher recommended puncture rate.

    Effect of multicomponent exercise in elderly patients with stable chronic obstructive pulmonary disease and frailty

    Objective To evaluate the effectiveness of multicomponent exercise in the elderly patients with stable chronic obstructive pulmonary disease(COPD) and frailty. Methods A multicomponent exercise intervention programme for the elderly with stable COPD and frailty was formed based on literature analysis and expert correspondence.Seventy-two elderly patients with stable COPD and frailty treated in Department of Geriatrics, Nanjing Drum Tower Hospital, from April 2022 to August 2023 were selected as the study subjects, and they were divided into the control group and the observation group according to the order of admission, with 36 cases in each group.The control group received conventional interventions, and the observation group added multi-component exercise interventions on the basis of the control group.The duration of the treatment was 12 weeks in both groups, and the results of pulmonary function indexes [forced expiratory volume in one second(FEV1), FEV1/forced vital capacity(FVC)], 6-minute walking distance, the scores of Modified Medical Research Council(mMRC), Fried frailty phenotype and Barthel index were compared between the two groups before and after the intervention. Results Before the intervention, there were no significant differences in all indicators between the two groups(P>0.05); After 12 weeks of the intervention, the difference in pulmonary function was not statistically significant(P>0.05), and the differences in 6-minute walking distance, mMRC score, Fried frailty phenotype score and Barthel index score between the two groups were all statistically significant(P<0.05). Conclusions For the elderly patients with stable COPD and frailty, adherence to regular multicomponent exercise is a valuable rehabilitation method, which can not only effectively reduce the degree of frailty, but also improve their exercise endurance and quality of life.

    Effects of discontinuing anticoagulation therapy on long-term prognosis in elderly patients with non-valvular atrial fibrillation

    Objective To evaluate the relationship between discontinuation of oral anticoagulants(OAC) use and clinical outcomes in the elderly patients with atrial fibrillation through long-term follow-up. Methods The elderly patients aged ≥75 years with non-valvular atrial fibrillation(NVAF) who had been prescribed OAC at Xuzhou Central Hospital from July 2018 to December 2020 were enrolled in this study. Demographic and clinical data of the patients were collected, and long-term follow-up was conducted through outpatient clinics and telephone calls. The composite outcome was stroke or all-cause death. Multivariate Cox regression model was used to analyze the relevant factors of clinical prognosis. Results A total of 560 patients with a median age of 80(78, 84) years were enrolled in the study. During a mean follow-up period of 33 months, 262 patients(46.8%) discontinued anticoagulant therapy. The discontinuation group had a significantly higher incidence of endpoint events during the follow-up period than the continuation group(46.9% vs. 25.2%,P<0.01). Multivariate Cox analysis showed that discontinuation of anticoagulant therapy was an independent risk factor for endpoint events(HR=1.74, 95%CI:1.29-2.34,P<0.01). Conclusions Nearly half of elderly NVAF patients who had received anticoagulation therapy discontinued OAC during long-term follow-up, and discontinuation of anticoagulation shows effects on poor clinical long-term prognosis.

    Correlation between intestinal dynamics and quality of life in patients with chronic obstructive pulmonary disease

    Objective To explore the correlation between intestinal dynamics and quality of life in the patients with chronic obstructive pulmonary disease(COPD). Methods Forty-five patients with COPD at the First Affiliated Hospital with Nanjing Medical University were enrolled by cross-sectional survey, and 45 healthy subjects were recruited from the society. The intestinal motility of the two groups was detected by gastrointestinal motility instrument, and their physiological and psychological status was evaluated by 36-item short form health survey(SF-36) questionnaire. Results The amplitude of ascending colon, transverse colon and the amplitude and frequency of descending colon in COPD group were higher than those in healthy group(P<0.05 or P<0.01). The scores of physical functioning(PF), role physical(RP), role emotional(RE), vitality(VT) and social functioning(SF) in SF-36 of the healthy group were significantly higher than those of COPD group(P<0.01). Intestinal motility was negatively correlated with PF, RE, SF, RP and VT in the patients with COPD (all P<0.05). Conclusions The intestinal motility in the patients with COPD is higher than that in healthy people. The disorder of intestinal dynamics shows a negative impact on the quality of life in the patients with COPD.

    Correlation between early dynamic changes of immune cells and pulmonary infection in elderly patients with acute respiratory distress syndrome

    Objective To explore the correlation between early immune cell dynamics and pulmonary infection in the elderly patients with acute respiratory distress syndrome(ARDS). Methods A total of 113 elderly patients with ARDS who were hospitalized in General Hospital of the Western Theater Command of Chinese PLA from January 2021 to January 2024 were enrolled in this study. They were divided into the infected group(49 cases) and the uninfected group(64 cases) based on whether they presented with pulmonary infection. The general information and early immune cell dynamics between the two groups were compared. Multivariate Logistic regression was used to analyze the effect of early dynamic changes of immune cells on the severity of pulmonary infection in the elderly patients with ARDS. Restricted cubic spline model was used to analyze the dose-response relationship between early immune cell dynamics and lung infection. Results There were statistically significant differences in the levels of C-reactive protein(CRP), procalcitonin(PCT) and white blood cell(WBC) between the two groups(P<0.05). Compared with the uninfected group, the level of CD3+ T cells was significantly increased and the levels of CD4+, CD8+ T cells and CD4+/CD8+ ratio were significantly decreased in the infected group 6 h, 2 d and 3 d after admission(P<0.05). Ordinal logistic regression analysis showed that the changes of CD4+, CD8+ T cells and CD4+/CD8+ had a certain effect on the degree of pulmonary infection in the elderly patients with ARDS(P<0.05). The correlation of CD3+, CD4+, CD8+cell levels and CD4+/CD8+ 6 h after admission with pulmonary infection in the elderly ARDS patients showed a non-linear dose-response relationship(Pnon-linearity=0.411, 0.516, 0.439, 0.402). Conclusions The early dynamic changes of immune cells in the elderly ARDS patients are closely related to pulmonary infection, and it has a certain effect on the degree of pulmonary infection.

    Correlation of serum levels of sclerostin and 25 hydroxyvitamin D with MMSE score in patients with Alzheimer's disease

    Objective To investigate the correlation of serum levels of sclerostin(SOST) and 25 hydroxyvitamin D[25(OH)D] with cognitive function in the patients with Alzheimer’s disease(AD). Methods A total of 30 patients with AD and 30 patients with mild cognitive impairment(MCI) treated in Xuzhou Central Hospital from January 2020 to December 2022, and 30 cases of age-, sex- and education-matched healthy controls during the same period were enrolled in this study. The serum levels of SOST and 25(OH)D in the three groups were detected and compared, and the correlation between serum levels of SOST, 25(OH)D and the score of mini-mental state examination(MMSE) in AD group was analyzed. Results The serum SOST level was significantly higher, and the serum 25(OH)D level was significantly lower in AD group than that in healthy control group(P<0.01). There were no significant differences in serum SOST and 25(OH)D levels between MCI group and healthy control group(all P> 0.05).Serum SOST level was negatively correlated with MMSE score(P<0.05), and 25(OH)D level was positively correlated with MMSE score in AD group(P<0.05). The results of multiple linear regression analysis showed that serum levels of 25(OH)D and SOST were the independent influencing factors of MMSE score in the AD patients. Conclusions Serum SOST level and 25(OH)D level are closely related to cognitive impairment in the AD patients, and can be potential therapeutic targets for AD.

    Objective To explore the relationship of serum uric acid(SUA) level with the decline in estimated glomerular filtration rate(eGFR) and the progression of chronic kidney disease(CKD) among the elderly. Methods A cohort of elderly adults in Foshan was established. The elderly individuals aged ≥65 years old who underwent health examinations in the Seventh Affiliated Hospital of Southern Medical University from January to December 2021 were enrolled in this study. The basic data of questionnaires, physical examinations, and laboratory indicators including blood lipids, blood glucose, and liver and kidney function were collected. All cases were followed for 2 years. The correlations of SUA level with eGFR and eGFR slope were analyzed by multiple linear regression analysis. Logistic regression model was used to investigate the influencing factors of the progression of CKD stage. Results With the increase of SUA level, eGFR significantly decreased(P<0.001), and the prevalence of CKD significantly increased(P<0.001). There was no statistical correlation between baseline SUA level and eGFR slope or CKD progression during the 2-year follow-up (P>0.05),but there was a significant correlation between the change of SUA level and eGFR slope or CKD progression(P<0.001). Logistic regression analysis showed that the change of SUA level was the risk factor for CKD progression. Conclusions In the elderly adults, SUA is significantly correlated with eGFR and the prevalence of CKD. The change of SUA level is significantly correlated with the decline of eGFR and the progression of CKD. Controlling the change of SUA level can prevent the occurrence and development of CKD.

    Correlation of fibrosis 4 score with Gensini score in elderly patients with acute coronary syndrome

    Objective To explore the relationship of fibrosis 4 score(FIB-4) with Gensini score in the elderly patients with acute coronary syndrome(ACS). Methods A total of 146 elderly patients with ACS were enrolled in the study,and were divided into the low Gensini group(≤37) and high Gensini group(>37) according to Gensini score. The general data and laboratory indexes were compared between the two groups. Multivariable logistic regression analysis was performed to identify the influencing factors of Gensini score. Receiver operating characteristic curve analysis was used to explore the predictive value of FIB-4 for the severity of coronary artery lesions in the patients with ACS. Results There were statistically significant differences in body mass index(BMI), white blood cell count, neutrophils, aspartate aminotransferase(AST), alanine-aminotransferase(ALT), total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), uric acid(UA), Gensini score, triglyceride-glucose index(TyG), FIB-4 and neutrophil-to-lymphocyte ratio(NLR) between the two groups. Multivariate logistic regression analysis showed that UA, TyG, FIB-4 and NLR were independent risk factors for the increase of Gensini score.The area under the curve of FIB-4 in predicting the increase of Gensini score was 0.79, which was higher than that of NLR, TyG and UA alone. Conclusions FIB-4 is positively correlated with Gensini score, and it can be used as economic and easily measured laboratory indicator to predict the severity of ACS in elderly patients.

    Development trajectory of cognitive function in elderly patients undergoing robot-assisted laparoscopic urological surgery

    Objective To explore the development trajectory of cognitive function in the elderly patients undergoing robot-assisted laparoscopic urological surgery. Methods The clinical data of the patients aged ≥65 years old receiving robot-assisted laparoscopic urological surgery were analyzed. The patients underwent the assessment of cognitive function by Mini-mental State Examination(MMSE) and Telephone Interview for Cognitive Status-Modified(TICS-m) scale.The development trajectory of postoperative short-term and long-term cognitive function was analyzed. Results The incidence rate of cognitive impairment in the elderly patients undergoing robot-assisted urological surgery 1, 3, 6 months after surgery was 26.7%, 19.9% and 14.3%, and the incidence rate of severe cognitive impairment was 6.2%, 4.7% and 4.4%. The TICS-m score in preoperative cognitive impairment group 3 months after surgery was 33.47±3.81, compared with 35.12±4.12 in normal preoperative cognitive function group(P=0.009). Compared with the patients with normal cognitive function 1 month after surgery, the patients with postoperative cognitive impairment 1 month after surgery presented with higher incidence rates of postoperative cognitive impairment 3, 6 months affter surgery. Conclusions Preoperative cognitive impairment in the elderly patients receiving robot-assisted urological surgery is associated with postoperative cognitive impairment 3 months after surgery. The occurrence of cognitive impairment 1 month after surgery has a warning effect on the occurrence of long-term cognitive impairment after surgery.

    Objective To explore the effects of moderate intensity aerobic exercise on elderly patients with mild to moderate post-stroke depression. Methods From May 2022 to May 2024, 90 elderly patients with post-stroke depression were recruited in Department of Rehabilitation of Jiangsu Province Official Hospital, and were randomly divided into control group(n=30), drug group(n=30) and aerobic exercise group(n=30) at a ratio of 1:1:1 by using Excel random number table method. The control group was given routine rehabilitation training, the drug group was given escitalopram combined with routine rehabilitation training, and the aerobic exercise group was given aerobic exercise combined with routine rehabilitation training. The intervention lasted for 4 weeks. Hamilton Depression Rating Scale(HDRS-24), Self-rating Depression Scale(SDS) and Barthel index were evaluated before and after the intervention. Results Compared with those before intervention, the scores of HDRS-24 and SDS in the aerobic exercise group were decreased after intervention(P<0.05), while Barthel index was increased(P<0.05). Compared with those in the control group and the drug group, the scores of HDRS-24 and SDS in the aerobic exercise group were decreased after intervention(P<0.05), and Barthel index was increased(P<0.05). Conclusions Moderate intensity aerobic exercise can effectively improve the depressive symptoms and improve the quality of life of the elderly patients with mild to moderate post-stroke depression.

    Effect of dietary nutrition literacy on nutritional risk of elderly people in the community

    Objective To investigate the association between dietary nutrition literacy and the nutritional risk among community-dwelling elderly adults. Methods A total of 118 elderly patients hospitalized at the community health service center and those receiving home care from April 2022 to December 2023 were enrolled in this study. After obtaining informed consent, the trained staffs conducted on-site surveys using dietary nutrition questionnaires and nutritional risk screening scales. Results There were no statistically significant differences in age, body mass index, education level, chronic conditions,the proportion of concerning the importance of balanced nutrition, self-perceived nutritional status, perceived importance of nutritional guidance, and willingness to participate in community-based nutritional interventions(all P>0.05), while there were significant differences in the proportion of awareness of the Dietary Guidelines Pyramid(P<0.01) between the elderly adults with nutritional risk and those without. Logistic regression analysis showed that being aware of the Dietary Guidelines Pyramid and being satisfied with one’s own nutrition status were the protective factors of nutritional risk among community-dwelling elderly adults. Conclusions Dietary nutrition literacy has a crucial impact on the nutritional risk among community-dwelling elderly adults.It is necessary to actively carry out health education on nutrition knowledge for the elderly in the future.

    Levels and diagnostic value of serum semaphorin 7A in elderly patients with acute coronary syndrome

    Objective To explore the levels and clinical value of serum semaphorin 7A (Sema7A) in the patients with acute coronary syndrome (ACS). Methods A total of 94 elderly patients with new onset of ACS who underwent coronary angiography from January 2020 to October 2023 in Fengyang County People’s Hospital were enrolled in ACS group, and 94 elderly patients with chronic coronary syndrome (CCS) undergoing coronary angiography at the same period were enrolled as control (CCS group). The level of serum Sema7A was evaluated by ELISA.The correlation between serum Sema7A level and the levels of interleukin-1β(IL-1β), IL-6, tumor necrosis factor-α (TNF-α) and IL-10 was analyzed.The level of serum Sema7A was compared between the two groups and its clinical significance was analyzed. Results The level of serum Sema7A was significantly higher in ACS group than that in CCS group (12.09±8.57 vs 8.65±6.57 ng/mL, P<0.01). According to Gensini score, the patients in ACS group were divided into mild disease group, moderate disease group and severe disease group.The level of serum Sema7A in severe disease group was significantly higher than that in moderate disease group and mild disease group(P<0.01).The level of serum Sema7A in ACS group was significantly correlated with the levels of IL-1β, IL-6, TNF-α and IL-10 (P<0.01). ROC curve analysis showed that the optimal cut-off value of serum Sema7A to predict ACS was 6.13 ng/mL with a sensitivity of 72.6%, a specificity of 78.3% and the area under curve (AUC) of 0.653 (95%CI:0.585-0.722, P<0.01). Conclusions Serum Sema7A level is significantly correlated with the severity of coronary artery stenosis in the elderly patients with ACS, and it may be a potential biomarker for predicting ACS.

    Cut-off value of grip strength and the relationship with intrinsic capability in Chinese elderly

    Objective To analyze the cut-off value of grip strength and the relationship with intrinsic ability and disability in Chinese elderly. Methods The data of 5 520 cases aged ≥ 60 years in the 2013 China Health and Elderly Longitudinal Survey were used to evaluate the levels of grip strength, intrinsic ability and disability of the elderly. Logistic regression model was used to analyze the relationship of grip strength with impairment in various domains of intrinsic ability and disability. Results The grip strength of men with different ages was greater than that of women. The cut-off points of weak grip strength in men and women aged 60-69 years, 70-79 years and ≥ 80 years were 10.1, 13.3, 18.8 kg and 6.9, 8.5, 10.2 kg, respectively. Overall, the risk of intrinsic capacity impairment in the elderly with weak grip strength was higher than that in the elderly with better grip strength(male: OR=2.382, 95%CI: 1.367-4.150; female:OR=5.514, 95%CI: 1.397-22.790),and the risk of disability was also higher in the elderly with weak grip strength than that in those with better grip strength (male:OR=2.582, 95%CI: 1.822-3.659; female: OR=1.820, 95%CI: 1.176-2.814). Conclusions The critical value of grip strength in Chinese elderly is low and needs to be improved. Grip strength is closely related to intrinsic ability and disability.

Brain metastases represent a significant factor contributing to the poor prognosis of the patients with lung cancer. Non-small cell lung cancer(NSCLC) is a common subtype of lung cancer.This review aims to comprehensively examine the correlation between epidermal growth factor receptor(EGFR) mutations and the occurrence of brain metastases in NSCLC patients, as well as the treatment strategies for those with EGFR-mutant NSCLC who present with brain metastases, especially the targeted therapy provided by the latest epidermal growth factor receptor tyrosine kinase inhibitors(EGFR-TKIs).

    Objective To explore the effects of Orem self-care theory on self-care ability, treatment compliance, mental state and quality of life in elderly patients with gastrointestinal cancer undergoing chemotherapy. Methods The elderly patients with gastrointestinal cancer who were hospitalized for chemotherapy were enrolled in this study. A total of 35 patients admitted from January to August 2023 were selected as the control group, and 35 patients admitted from September 2023 to March 2024 were selected as the observation group. The control group was given routine care, and the observation group was given Orem self-care theory on the basis of the control group. The self-care ability(exercise of self-care agency scale, ESCA), treatment compliance(Frankl compliance scale), mental state(self-rating anxiety scale, SAS; self-rating depression scale, SDS) and quality of life(short form heath survey, SF-36) of the two groups were compared. Results After intervention, the scores of ESCA, Frankl compliance scale and SF-36 in the two groups were higher than those before intervention, and the scores in the observation group were higher than those in the control group(P<0.05). The SAS and SDS scores in the two groups were lower than those before intervention(P<0.05), and those in the observation group were lower than those in the control group(P<0.05). Conclusions Orem self-care theory nursing intervention for elderly patients with gastrointestinal cancer undergoing chemotherapy during hospitalization may improve the self-care ability, treatment compliance, mental state and quality of the life of patients.

    The early stage of Parkinson’s disease is a key period to improve symptoms and delay the disease progression. Levodopa/Carbidopa/Entacapone is one of the recommended drugs for the treatment of early Parkinson’s disease in the guidelines. To better standardize and guide the application of levodopa/carbidopa/entacapone in early Parkinson’s disease, the writing group has conducted a thorough analysis of both domestic and international evidence-based medicine, taking into account of the specific circumstances of clinical practice in China, and jointly discussed and formulated this expert advice. The content encompasses the optimization effects of levodopa/carbidopa/entacapone on the pharmacokinetics of traditional compound levodopa, the efficacy and safety of these medications for patients with early Parkinson’s disease, the target population, dosing regimens, management of common adverse reactions and treatment adherence, aiming to provide references for clinical standardization and rational drug use.
