老年髋关节骨折病人术后恢复水平与肌少症的相关性

Correlation between postoperative recovery level and sarcopenia in elderly patients with hip fractures

摘要: 目的 探讨影响老年髋关节骨折病人术后恢复的主要因素,为改善老年髋关节骨折病人的预后提供参考。 方法 本研究共纳入129例老年髋关节骨折病人,其中男72例,女57例。采用Harris评分评价术后恢复情况,采用肌肉力量测试诊断肌少症,并采用Logistic回归分析肌少症及其他指标对老年髋关节骨折病人术后恢复的影响。 结果 单因素分析显示,Harris评分≥80分组骨骼肌质量指数(SMI)、骨骼肌、无机盐、维生素D水平及肌少症与Harris评分<80分组比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,老年髋关节骨折病人术后恢复与肌少症、维生素D和无机盐含量有关。 结论 老年髋关节骨折术后应针对肌少症和相关因素进行干预,以缩短髋关节术后的恢复时间、提高恢复效果。

Abstract: Objective To identify the main factors affecting the postoperative recovery in the elderly patients with hip fractures and provide recommendations to improve the prognosis. Methods A total of 129 elderly patients with hip fractures were enrolled in this study including 72 males and 57 females. Harris score was used to evaluate the postoperative recovery, and muscle strength testing was used to diagnose sarcopenia. Logistic regression was used to assess the correlation of sarcopenia and other indicators with postoperative recovery in the elderly patients with hip fracture. Results Univariate analysis showed there were statistically significant differences in skeletal muscle mass index(SMI), skeletal muscle mass, the levels of inorganic salt and vitamin D, sarcopenia between Harris score ≥80 group and Harris score <80 group(P<0.05). Multivariate Logistic regression analysis showed that sarcopenia, vitamin D and inorganic salt content were independent risk factors of postoperative recovery in the elderly patients with hip fracture. Conclusions It is important to intervene sarcopenia and related factors in the elderly patients with hip fracture postoperatively to shorten the recovery time and improve the overall recovery outcomes.
