社区老年人群心理健康因子与其心理问题的关系分析

Relationship between mental health factors and psychological problems among community-dwelling elderly people

摘要: 目的 系统分析“认知-情绪-适应-性格-人际”多维度心理健康因子与社区老年人常见心理问题的关联性,量化各因子的影响强度,为精准化心理健康干预提供理论依据。 方法 采用分层多阶段抽样法,于2023年1—6月抽取山东省8个社区的570例老年人为研究对象。使用艾森克情绪稳定量表(修订版)、UCLA孤独量表(第3版)及老年人心理健康量表进行测评,采用Spearman秩相关筛选与心理问题相关的因子,建立逐步多元线性回归模型,分析心理健康因子与老年人常见心理问题的关联特征及影响强度。 结果 社区老年人常见心理问题检出率从高到低依次为:疑心病观念(91.93%)、负罪感(90.88%)、依赖性(89.30%)、强迫性(78.95%)、抑郁性(73.68%)、自卑感(60.70%)和焦虑(38.95%)。84.21%的社区老年人存在中高度孤独感。多元线性回归分析显示:人际因子是焦虑、负罪感、强迫性的唯一显著影响因子(β=-0.23、-0.20、-0.14,P均<0.001),同时对抑郁性、疑心病观念、自卑感、孤独感均具有显著保护作用;适应因子对疑心病观念(β=-0.20,P<0.001)与自卑感(β=-0.20,P<0.001)的影响最强,并可有效缓解孤独感;情绪因子与性格因子可显著降低依赖性(β=0.15、0.19,P均<0.001),改善抑郁性与自卑感;认知因子未进入任何心理问题的回归模型,无独立显著影响。 结论 社区老年人心理健康问题以健康担忧、自我价值贬抑、依赖倾向与孤独感较为突出,人际因子是负性心理问题最核心的保护因素,适应、情绪与性格因子亦发挥重要作用。应构建以人际支持为核心、适应训练为重点、情绪性格培育为基础的精准化、多层次心理健康干预体系,强化城乡服务均衡供给,全面提升社区老年人群心理健康水平。

Abstract: Objective To systematically analyze the association between multi-dimensional mental health factors (cognition, emotion, adaptation, personality, and interpersonal relationship) and common psychological problems among community-dwelling elderly people, and quantify the influence intensity of each factor, so as to provide a theoretical basis for targeted mental health interventions. Methods A stratified multistage sampling method was used to select 570 elderly individuals from 8 communities in Shandong Province from January to June 2023. The revised Eysenck Emotional Stability Scale, the 3rd edition of the UCLA Loneliness Scale, and the Mental Health Scale for the Elderly were used for evaluation. Spearman rank correlation was used to screen factors significantly associated with psychological problems, and a stepwise multiple linear regression model was established to analyze the association characteristics and influence intensity between mental health factors and common psychological problems in the elderly. Results The detection rates of common psychological problems in the community-dwelling elderly were as follows: hypochondriasis (91.93%), guilt (90.88%), dependence (89.30%), obsessiveness (78.95%), depression (73.68%), inferiority (60.70%), and anxiety (38.95%). A total of 84.21% of the participants had moderate to high loneliness. Multiple linear regression showed that interpersonal factor was the only significant predictor for anxiety, guilt and obsessiveness (β=-0.23, -0.20, -0.14, all P<0.001), and also had significant protective effects on depression, hypochondriasis, inferiority and loneliness. Adaptation factor had the strongest effects on hypochondriasis (β=-0.20, P<0.001) and inferiority (β=-0.20, P<0.001), and could effectively alleviate loneliness. Emotional and personality factors significantly reduced dependence (β=0.15, 0.19, both P<0.001), and improved depression and inferiority. Cognitive factor was not included in any regression model and had no independent significant effect. Conclusions Psychological problems of community-dwelling elderly people are mainly characterized by health anxiety, self-deprecation, dependence and loneliness. Interpersonal factor is the core protective factor against negative psychological problems, and adaptation, emotion and personality factors also play important roles. A targeted and multi-level mental health intervention system should be constructed with interpersonal support as the core, adaptation training as the focus, and emotional and personality cultivation as the basis, so as to strengthen the balanced supply of urban and rural services and comprehensively improve the mental health of community-dwelling elderly people.
