KIR2DL4基因多态性及促凋亡基因HtrA2在老年白血病病人中的表达及与预后的相关性研究

KIR2DL4 gene polymorphism and pro-apoptotic gene HtrA2 expression and their correlation with prognosis in elderly patients with leukemia

摘要: 目的 探讨KIR2DL4基因多态性及促凋亡基因HtrA2在白血病病人中的表达及与预后的相关性。方法 选择2016年7月至2018年1月收治的老年白血病病人156例作为研究对象,设为观察组,其中急性淋巴细胞白血病(ALL)67例,急性髓系白血病60例,慢性髓系白血病(CML)29例,所有病人均给予常规方法治疗, 随访2年。选择同期健康体检的老年人79例,设为对照组。完成基因组DNA提取,检测KIR2DL4基因分型;采用实时荧光PCR技术测定2组促凋亡基因HtrA2 mRNA水平;对KIR2DL4基因多态性及促凋亡基因HtrA2表达与老年白血病病人预后的相关性进行分析。结果 2组KIR2DL4基因多态性中2DL4-001、2DL4-005、2DL4-006、2DL4-008、2DL4-0011差异无统计学意义(P>0.05);观察组9A型KIR2DL4高于对照组(P<0.05);10A型KIR2DL4低于对照组(P<0.05)。观察组HtrA2 mRNA水平高于对照组(P<0.05)。随访期间死亡54例,死亡率为34.62%。死亡组2DL4-0011、9A型KIR2DL4检出率均高于存活组(P<0.05),10A型KIR2DL4检出率和HtrA2表达水平低于存活组(P<0.05)。多因素Cox回归分析结果表明:老年白血病病人预后与2DL4-006、9A型KIR2DL4基因多态性及HtrA2表达水平均有关(P<0.05)。结论 KIR2DL4基因多态性及促凋亡基因HtrA2在老年白血病病人中表达异常,其水平与预后存在相关性。

Abstract: Objective To investigate the expression of KIR2DL4 gene polymorphism and pro-apoptotic gene HtrA2 in the elderly patients with leukemia and their correlation with prognosis. Methods A total of 156 elderly patients with leukemia from July 2016 to January 2018 were selected as the observation group. Among the patients, 67 cases were acute lymphoblastic leukemia (ALL), 60 cases were acute myeloid leukemia, and 29 cases were chronic myeloid leukemia (CML). All patients were given conventional treatment, and completed a 2-year follow-up. There were 79 cases of elderly people who underwent healthy physical examinations during the same period were selected as control groups. All the subjects complete genomic DNA extraction, and specific PCR amplification of all coding regions; Real-time fluorescent PCR technology was used to determine the expression of pro-apoptotic genes HtrA2 mRNA. The correlation between the KIR2DL4 gene polymorphism and the expression of gene HtrA2 with the prognosis of leukemia was analyzed. Results There were no statistically significant in 2DL4-001, 2DL4-005, 2DL4-006, 2DL4-008 and 2DL4-0011 gene polymorphisms between the two groups(P>0.05). The gene polymorphisms of 9A type KIR2DL4 were higher and 10A KIR2DL4 was lower in the observation group than those in the control group (P<0.05). The expression of HtrA2 mRNA in the observation group was higher than that in the control group (P<0.05). During the follow-up period, 54 patients died, with a mortality rate of 34.62%. The detection rates of type 2DL4-0011 and type 9A Kir2DL4 in death group were higher, while the detection rate of type 10A Kir2DL4 and the expression of HtrA2 were lower than those in survival group (P<0.05). Cox regression analysis showed that the prognosis of patients with senile leukemia was related to the polymorphism of 9A Kir2DL4, 2DL4-006 and the expression of HtrA2(P<0.05). Conclusions KIR2DL4 gene polymorphism and pro-apoptotic gene HtrA2 are abnormally expressed and correlated with prognosis in the elderly patients with leukemia.
