AMPKα1/TLR4通路在老年糖尿病肾病病人中的表达及与NF-κB、TGF-β1水平的相关性研究

Expression of AMPKα1/TLR4 pathway in elderly patients with diabetic nephropathy and the correlation with NF-κB and TGF-β1 levels

摘要: 目的 探讨腺苷酸活化蛋白激酶α1(AMPKα1)/Toll样受体4(TLR4)通路在老年糖尿病肾病病人中的表达及与外周血核因子-κB(NF-κB)、转化生长因子-β1(TGF-β1)水平的相关性。方法 选择2017年3月至2019年5月老年糖尿病肾病病人126例作为对象,设为观察组,所有病人均经肾脏活检确诊;选择同期行肾脏切除术老年病人(非糖尿病肾病)81例,设为对照组。采用实时荧光PCR法、HE染色测定不同组织中AMPKα1/TLR4 mRNA水平;采用ELISA法测定2组NF-κB、TGF-β1水平;采用Pearson相关系数对观察组AMPKα1/TLR4 mRNA水平与NF-κB、TGF-β1水平进行相关性分析。结果 观察组AMPKα1/TLR4 mRNA水平均低于对照组(P<0.01)。HE染色结果表明,观察组肾小球毛细血管基底弥漫性增厚,伴有纤维化,且小动脉管壁内膜增厚;而对照组肾小球及肾小管均未见明显病变,亦未见纤维化。观察组NF-κB、TGF-β1水平均高于对照组(P<0.05); Pearson相关性分析结果表明,观察组AMPKα1/TLR4表达与NF-κB、TGF-β1水平均呈负相关性(P<0.05)。结论 AMPKα1/TLR4、NF-κB以及TGF-β1在老年糖尿病肾病病人中表达异常,可能直接参与疾病的发生、发展。

Abstract: Objective To investigate the expression of AMPKα1 / TLR4 pathway in the elderly patients with diabetic nephropathy and the correlation with the levels of nuclear factor-κB (NF-κB) and transforming growth factor-β1 (TGF-β1). Methods A total of 126 elderly patients with diabetic nephropathy confirmed by renal biopsy from March 2017 to May 2019 were selected as the observation group; And 81 elderly patients (non diabetic nephropathy) who underwent nephrectomy at the same time were selected as the control group. The levels of AMPKα 1 / TLR4 mRNA in different tissues were measured by real-time polymerase chain reaction and hematoxylin-eosin (HE) staining, and the levels of NF-κB and TGF-β1 in the two groups were measured by enzyme-linked immunosorbent assay. The correlations between AMPKα1 / TLR4 mRNA and NF-κB, TGF-β1 were analyzed by Pearson correlation coefficient. Results The level of AMPKα1/TLR4 mRNA in the observation group was lower than that in the control group (P<0.01). HE staining showed that the glomerular capillary basement in the observation group was diffusely thickened, accompanied with fibrosis, and the intima of small artery wall was thickened; While in the control group, the glomeruli and renal tubules showed no obvious pathological changes and no fibrosis. The levels of NF-κB and TGF-β1 in the observation group were higher than those in the control group. Pearson correlation analysis showed that AMPK α1 / TLR4 mRNA level was negatively correlated with NF-κB and TGF-β1 levels in the elderly patients with diabetic nephropathy (P < 0.05). Conclusions The expression of AMPK α1 / TLR4, NF-κB and TGF-β1 are abnormal in the elderly patients with diabetic nephropathy. They might directly participate in the occurrence and development of the disease.
