脊柱小关节微调对老年非特异性下腰痛小关节紊乱亚型的疗效研究

Clinical efficacy of spinal facet joint micro-adjustment in elderly patients with non-specific low back pain of the facet joint dysfunction subtype

摘要： 目的 观察脊柱小关节微调治疗对老年非特异性下腰痛小关节紊乱亚型患者的临床疗效。方法 选择2025年3—6月于南京医科大学附属老年医院就诊的老年非特异性下腰痛小关节紊乱亚型患者60例作为研究对象,以信封法随机分成观察组(n=30)和对照组(n=30)。2组患者均接受电脑中频治疗,在此基础上,对照组采取脊柱小关节假微调干预,观察组采取IQ脉冲治疗枪辅助脊柱小关节微调干预,干预频率均为1周1次,持续2周。比较2组治疗前后疼痛与功能量表评分[数字疼痛分级量表(NRS)、Oswestry功能障碍指数(ODI)、日常生活活动能力(ADL)评分]、足底压力与步态分析指标的差异,并记录治疗期间不良反应的发生情况。结果 治疗后,与对照组比较,观察组患者NRS评分、ODI评分、足底峰值压力均更低(P＜0.05),ADL评分及髋关节、膝关节的关节活动度均更高(P＜0.05)。治疗期间2组无明显不良反应发生。结论 IQ脉冲治疗枪辅助脊柱小关节微调治疗老年非特异性下腰痛小关节紊乱亚型短期疗效确切,可有效缓解疼痛、改善腰部功能及日常生活能力,优化足底压力分布及步态,且安全性良好。

Abstract: Objective To investigate the clinical efficacy of spinal facet joint micro-adjustment assisted by IQ pulse therapy gun in elderly patients with non-specific low back pain(NLBP) of the facet joint dysfunction subtype. Methods A total of 60 elderly patients with NLBP of facet joint dysfunction subtype, who were treated at Geriatric Hospital of Nanjing Medical University from March to June 2025, were selected and randomly divided into an observation group(n=30) and a control group(n=30) using the envelope randomization method. Both groups received computerized medium-frequency electrotherapy. In addition, the control group underwent sham spinal facet joint micro-adjustment, while the observation group received spinal facet joint micro-adjustment assisted by an IQ pulse therapy gun. Interventions were performed once weekly for 2 consecutive weeks in both groups. The scores of pain and functional scales [Numerical Rating Scale(NRS), Oswestry Disability Index(ODI), Activities of Daily Living(ADL) scale], plantar pressure and gait analysis indicators were compared between the two groups before and after treatment. Adverse reactions during the treatment were also recorded. Results After treatment, the observation group exhibited significantly lower NRS scores, ODI scores and peak plantar pressure values than the control group(P＜0.05). Meanwhile, ADL scores, hip and knee joint range of motion(ROM) were significantly higher than those in the control group(P＜0.05). No obvious adverse reactions occurred in either group during the treatment period. Conclusions Spinal facet joint micro-adjustment assisted by IQ pulse therapy gun yields satisfactory efficacy in elderly patients with NLBP of the facet joint dysfunction subtype. This intervention can effectively alleviates pain, improves lumbar function and ADL, and optimizes plantar pressure distribution and gait patterns.
