Abstract: Frailty is a geriatric syndrome characterized by multisystem functional decline. Against the backdrop of accelerating population aging, its coexistence with cardiometabolic multimorbidity in the elderly population has become increasingly prominent, with the two conditions reinforcing each other and exacerbating clinical outcomes. This article aims to systematically review the shared pathophysiological network between frailty and cardiometabolic multimorbidity, and to explore the intertwined biological mechanisms underlying their interaction. Clinical studies have identified chronic inflammation as the core driver of this network. Various pathological mechanisms, including cellular senescence, gut microbiota dysbiosis, neuroendocrine dysregulation, insulin resistance, mitochondrial dysfunction, and obesity, ultimately contribute to the development of both frailty and multiple cardiometabolic diseases by exacerbating chronic inflammation. Elucidating this complex mechanistic network not only helps to clarify the close etiological link between the two but also provides a theoretical basis for early clinical identification and comprehensive intervention, thereby improving health management in the elderly population.
