肌少症对老年抑郁症患者情景记忆功能的影响

Effect of sarcopenia on episodic memory function in patients with late-life depression

摘要: 目的 探讨肌少症与老年抑郁症(LLD)患者情景记忆功能的关联。 方法 于2024年5月至2025年2月在合肥市第四人民医院招募101例老年抑郁症患者。依据亚洲肌少症工作组标准,将LLD患者分为伴肌少症组(LLD-S组,40例)与不伴肌少症组(LLD-NS组,61例)。采用听觉词语学习测验-华山版评估记忆功能,检测肌少症相关躯体指标(肌肉力量、躯体功能)并完成生物电阻抗分析;应用汉密尔顿抑郁量表、汉密尔顿焦虑量表及MMSE量表评估精神心理症状。采用Spearman相关分析评估情景记忆功能与相关临床指标间的关联;构建分层线性回归模型,分析肌少症对LLD患者情景记忆功能的独立影响。 结果 LLD-S组较LLD-NS组年龄更大、躯体功能更差、抑郁焦虑更重,且认知与记忆损害更明显(P<0.05)。LLD患者短延迟回忆、长延迟回忆及再认能力与5次起坐试验时间、HAMD和HAMA评分均呈负相关(P<0.05),与握力、小腿围、6 m步速均呈正相关(P<0.01)。分层线性回归模型结果显示,在校正人口学、共病及精神症状等因素后,肌少症对老年抑郁症患者的长延迟回忆(B=-1.36, P=0.001)、短延迟回忆(B=-1.03, P=0.014)及再认能力(B=-1.92, P=0.043)均有显著负向影响。 结论 肌少症与LLD患者情景记忆功能障碍存在密切关联,且是LLD患者情景记忆功能损害的独立危险因素。

Abstract: Objective To investigate the association between sarcopenia and episodic memory function in patients with late-life depression (LLD). Methods A total of 101 elderly patients with depression were recruited from May 2024 to February 2025. According to the Asian Working Group for Sarcopenia criteria, the patients were divided into a group with sarcopenia (LLD-S group,n=40) and a group without sarcopenia (LLD-NS group, n=61). The Auditory Verbal Learning Test-Huashan version was used to assess memory function. Sarcopenia indicators including muscle strength and physical function were measured and bioelectrical impedance analysis was performed. Hamilton Depression (HAMD) scale, Hamilton Anxiety(HAMA) scale, and Mini-Mental State Examination (MMSE) were applied to evaluate the mental and psychological state. Spearman correlation analysis was employed to evaluate the association between episodic memory function and relevant clinical indicators; And hierarchical linear regression model was constructed to analyze the independent impact of sarcopenia on episodic memory function in LLD patients. Results The LLD-S group exhibited higher age, poorer physical function, more severe depression and anxiety, as well as more pronounced cognitive and memory impairments compared to the LLD-NS group (all P<0.05). In LLD patients, short-delay recall, long-delay recall, and recognition were all negatively correlated with the duration of the five-chair-standing test, as well as HAMD and HAMA scores (P<0.05), while significantly positively correlated with grip strength, calf circumference, and 6-meter walking speed. Hierarchical regression showed that sarcopenia had significant negative effects on long-delay recall (B=-1.36, P=0.001), short-delay recall (B=-1.03, P=0.014), and recognition (B=-1.92, P=0.043). Conclusions Sarcopenia is closely associated with impaired episodic memory function in patients with LLD, and it is an independent risk factor for episodic memory impairment in LLD patients.
