六字诀联合弹力带训练对老年慢性阻塞性肺疾病稳定期病人肺康复的效果

Effect of Liu Zi Jue exercise combined with elastic belt resistance training on pulmonary rehabilitation in elderly patients with chronic obstructive pulmonary disease at stable stage

摘要: 目的 探讨六字诀联合弹力带训练改善老年COPD稳定期病人的肺功能和运动耐力的效果。方法 选取2019年6月至2021年6月南京中医药大学附属医院老年科、呼吸科收治的60例老年COPD稳定期病人,根据计算机软件产生的随机数将其随机分为对照组和试验组,每组30例。对照组在规范治疗的基础上进行弹力带训练,试验组在对照组的基础上进行六字诀呼吸操训练,2组均每周训练5次,30 min/次,干预6个月。分别于干预前及干预3、6个月时,评价2组病人的肺功能(FEV1、FVC和FEV1/FVC)、6 min步行距离(6MWD)、慢性阻塞性肺疾病评估测试(CAT)评分。结果 干预3个月,2组FEV1、FVC、FEV1/FVC、6MWD、CAT评分均较干预前显著改善(P<0.05),且FEV1、FVC、FEV1/FVC、CAT评分组间差异有统计学意义(P<0.05);干预6个月,2组病人FEV1、FVC、FEV1/FVC、6MWD、CAT评分均优于干预前及干预3个月时,且上述指标组间差异均有统计学意义(P<0.05)。结论 六字诀结合弹力带训练的锻炼方式有助于改善老年COPD稳定期病人的肺功能,提高运动耐力,改善生活质量。

Abstract: Objective To investigate the effect of Liu Zi Jue exercise combined with elastic belt resistance training on pulmonary function and quality of life in the elderly patients with chronic obstructive pulmonary disease (COPD) at stable stage. Methods A total of 60 elderly COPD patients at stable stage admitted to Department of Geriatrics and Department of Respiratory Medicine of Affiliated Hospital of Nanjing University of Chinese Medicine from June 2019 to June 2021 were selected. According to the random numbers generated by computer software, all the patients were randomly divided into the control group and the experimental group, with 30 cases in each group. The control group received elastic belt resistance training on the basis of standardized treatment, and the experimental group received Liu Zi Jue breathing exercise on the basis of the control group. Both groups were trained 5 times a week (30 min each time) for 6 months. The pulmonary function [forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC) and FEV1/FVC], 6-min walking distance (6MWD), and Chronic Obstructive Pulmonary Disease Assessment Test (CAT) scores of the patients were evaluated before the intervention and after 3, 6 months of the intervention, respectively. Results After 3 months of the intervention, the levels of FEV1, FVC, FEV1/FVC and 6MWD, and the score of CAT in both groups were significantly improved (P<0.05), and the levels of FEV1, FVC and FEV1/FVC, and the score of CAT showed significant differences between the two groups (P<0.05). After 6 months of the intervention, the levels of FEV1, FVC, FEV1/FVC and 6MWD, and the score of CAT in both groups were significantly imporved compared with those after 3 months of the intervention, and differences between the two groups were significant (P<0.05). Conclusions Liu Zi Jue exercise combined with elastic belt resistance training can effectively increase exercise endurance, improve pulmonary function and quality of life of the elderly COPD patients at stable stage.
