老年听力障碍药物靶点的研究进展

Research progress on drug targets for age-related hearing loss

摘要: 老年听力障碍也称年龄相关性听力损失(age-related hearing loss, ARHL),是全球范围内最普遍的慢性退行性疾病之一,严重影响老年人的生活质量及心理健康,然而目前临床上尚无有效的预防及治疗药物。本文从ARHL发生发展的三大核心机制(氧化应激、线粒体功能障碍及慢性炎症)角度,系统介绍相关药物靶点的最新研究进展。虽然基于这些靶点研发的药物已在基础实验中展现出耳蜗保护及延缓听力损失的潜在疗效,但仍存在实验模型多脱离人类自然衰老进程、药物耳蜗靶向递送效率低、候选药物长期安全性与老年生理环境下的疗效尚未充分验证的局限性。

Abstract: Age-related hearing loss (ARHL), also known as presbycusis, is one of the most prevalent chronic degenerative diseases worldwide, seriously affecting the quality of life and mental health of the elderly. However, there are currently no effective preventive or therapeutic drugs in clinical practice. This review systematically summarizes the latest research progress on potential drug targets from the perspective of three core mechanisms underlying the occurrence and development of ARHL: oxidative stress, mitochondrial dysfunction, and chronic inflammation. Finally, it proposes that although drugs developed based on these targets have demonstrated potential efficacy in protecting the cochlea and delaying hearing loss in preclinical studies, there are still limitations such as most experimental models being divorced from the natural human aging process, low efficiency in targeted drug delivery to the cochlea, and insufficient validation of the long-term safety of candidate drugs and their efficacy in the elderly physiological environment.
