血清硫酸脱氢表雄酮和年龄的比值与老年男性糖尿病肾病的相关性分析

Correlation between serum dehydroepiandrosterone sulfate-to-age ratio and diabetic kidney disease in elderly male patients with type 2 diabetes mellitus

摘要: 目的 分析老年男性T2DM病人血清硫酸脱氢表雄酮和年龄的比值(DHEAS/AGE)与糖尿病肾病(DKD)的相关性。 方法 纳入南京鼓楼医院老年科2021—2023年诊治的195例老年T2DM病人,根据是否合并DKD分为DKD组和非DKD组。比较2组病人一般资料、血清DHEAS水平及校正年龄的DHEAS/AGE的变化,探究DHEAS/AGE与代谢指标、肾功能指标以及DKD患病风险间的关系。 结果 DKD组DHEAS/AGE显著低于非DKD组(P<0.05)。DHEAS/AGE与年龄、高血压、尿白蛋白/肌酐比值(UACR)呈显著负相关,与估算肾小球滤过率(eGFR)呈正相关(均P<0.05)。按DHEAS/AGE四分位数将病人分为Q1~Q4 4组,与其对应的DKD患病率分别为41.7%、20.8%、28.0%和16.3%(趋势P<0.05)。将Q1组病人DKD的患病风险(OR)设为1.00,校正年龄、BMI、SBP等因素后,Q2、Q3、Q4组病人患DKD的风险[OR(95%CI)]分别为0.267(0.097~0.732)、0.369(0.139~0.978)、0.122(0.036~0.415)(趋势P<0.05)。 结论 DHEAS/AGE降低与老年男性T2DM病人DKD的患病风险增加显著相关。

Abstract: Objective To evaluate the correlation between the serum dehydroepiandrosterone sulfate-to-age ratio (DHEAS/AGE) and diabetic kidney disease (DKD) in the elderly male patients with type 2 diabetes mellitus (T2DM). Methods A total of 195 elderly male T2DM patients treated in Nanjing Drum Tower Hospital from 2021 to 2023 were enrolled in the study. All patients were divided into DKD group and non-DKD group. The general information, serum DHEAS level, DHEAS/AGE were compared between the two groups. The relationship of DHEAS/AGE with metabolic parameters, renal function parameters, and the risk of DKD were analyzed by correlation analysis and unconditional Logistic regression analysis. Results DHEAS/AGE in DKD group was significantly lower than that in non-DKD group (P<0.05). DHEAS/AGE was significantly negatively correlated with age, hypertension, urinary albumin creatinine ratio (UACR), and significantly positively correlated with estimated glomerular filtration rate (eGFR) (all P<0.05). All patients were divided into 4 groups according to DHEAS/AGE quartiles from low to high (Q1 to Q4), and the corresponding incidence rate of DKD was 41.7%, 20.8%, 28.0%, and 16.3% (Ptrend<0.05), respectively. After adjusting the factors such as age, body mass index and hypertension, when compared with the Q1 group (reference, 1.00), the risk of patients with DKD in Q2, Q3 and Q4 groups were 0.267(0.097-0.732), 0.369(0.139-0.978) and 0.122(0.036-0.415)(Ptrend<0.05), respectively. Conclusions DHEAS/AGE reduction is significantly correlated with an increased risk of DKD in the elderly male patients with T2DM.
