三种药物联合高能红光治疗老年带状疱疹的疗效观察及其对炎性因子的影响

Effects of three drugs combined with high-energy red light irradiation in the treatment of herpes zoster in the elderly

摘要: 目的 评估伐昔洛韦、普瑞巴林、甲钴胺三种药物联合高能红光照射治疗老年带状疱疹(HZ)的临床疗效,并探讨其可能的作用机制。方法 90例老年HZ急性发作期病人按随机数表法分成试验组和对照组,每组45例。对照组仅使用药物治疗(伐昔洛韦、普瑞巴林和甲钴胺),试验组在此基础上加用高能红光照射治疗。分别于治疗前、治疗7 d后、治疗4周后记录病人VAS评分,检测血清中IL-2、IL-6和TNF-α水平,评估临床疗效并进行比较分析。结果 2组病人治疗7 d及4周后VAS评分均比治疗前显著降低(P<0.05),且试验组较对照组改善更显著(P<0.05)。与治疗前相比,2组治疗7 d及4周时的血清IL-2水平均升高,IL-6和TNF-α水平均降低(P<0.05),且试验组改善更显著(P<0.05)。试验组有效率为86.67%,明显高于对照组的66.67%(P<0.05)。结论 伐昔洛韦、普瑞巴林、甲钴胺三种药物联合高能红光照射治疗老年HZ疗效更佳,其作用机制可能与调节免疫应答和降低炎症反应相关。

Abstract: Objective To evaluate the clinical efficacy of valacyclovir, pregabalin and mecobalamin combined with high-energy red light irradiation in the treatment of herpes zoster (HZ) in the elderly, and to explore the possible mechanism. Methods A total of 90 elderly patients with acute HZ were randomly divided into the experimental group and the control group, with 45 cases in each group. The control group was treated with drugs as valacyclovir, pregabalin and mecobalamin, and the experimental group was treated with high-energy red light irradiation on the basis of the drug treatment.The score of Visual Analogue Scale (VAS), serum levels of interleukin-2(IL-2), IL-6 and tumor necrosis factor-α (TNF-α) before treatment, 7 days and 4 weeks after treatment were detected, and the clinical efficacy of the two groups was assessed and compared. Results The VAS scores of the two groups after 7 days and 4 weeks of treatment were significantly lower than those before treatment (P<0.05), especially in the experimental group (P<0.05). Compared with those before treatment, the levels of IL-2 7 days and 4 weeks after treatment were increased, and the levels of IL-6 and TNF-α were decreased in the two groups(P<0.05), especially in the experimental group (P<0.05). The effective rate in the experimental group was 86.67%, compared with 66.67% in the control group (P<0.05). Conclusions Valacyclovir, pregabalin and mecobalamin combined with high-energy red light irradiation is more effective in the treatment of the elderly patients with HZ, and its mechanism may be related to the regulation of immune response and the reduction of inflammation.
