    Application of a remote visitation system for elderly inpatients with cancer during the novel coronavirus infection epidemic period

    Objective To explore the application value of a remote visiting system in the elderly inpatients with cancer during the novel coronavirus infection epidemic period. Methods A total of ninety-two elderly patients with cancer in our oncology ward from April to October 2022 were enrolled, and video visits were conducted using the remote visitation system weblink App. The cognitive function, daily living abilities, degree of anxiety and depression, sleep quality and the patients family satisfaction before and after the visits were observed and compared. Results After the remote visitation, the scores of depression and anxiety were lower and sleep quality was improved,and the satisfaction of the family was higher than that before the remote visitation (P<0.01). There were no significant differences in the MMSE and Barthel Index scores before and after the remote visitation. Conclusions The application of remote visitation systems during the novel coronavirus infection epidemic period not only meets the communication between inpatients and their families but also improves sleep quality and relieve anxiety and depression of the patients, and improves the satisfaction of the patients and the family.

    Correlation of serum uric acid level with metabolic syndrome in elderly aged≥80 years old

    Objective To explore the relationship of the serum uric acid(UA) level with metabolic syndrome(MS) in the elderly aged ≥80 years old. Methods A total of 1013 elderly aged ≥80 years old from the Health Management Center of Jiangsu Provincial Geriatric Hospital were enrolled in this study.The serum level of UA was detected, and the MS were evaluated. Pearson correlation and Logistic regression were used to explore the relationship of serum UA level with MS. Results Among 1013 elderly aged≥80 years old, there were 269 (26.6%) cases of hyperuricemia, 210(20.7%) cases of MS. The level of serum UA was positively correlated with age, body mass index, triglycerides and MS, while negatively correlated with fasting plasma glucose and high-density lipoprotein cholesterol. High level of UA was an independent risk factor for MS. Conclusions In the elderly aged ≥80 years old, the level of UA is independently related to MS.

    Effects of thoracic paravertebral block on postoperative pulmonary complications in elderly patients undergoing thoracoscopic partial lung resection

    Objective To explore the effects of thoracic paravertebral block (TPVB) on postoperative pulmonary complications (PPCs) in the elderly patients undergoing thoracoscopic partial lung resection based on propensity score matching method. Methods A retrospective analysis of 818 elderly patients who underwent thoracoscopic partial lung resection from October 2021 to June 2022 was conducted. The patients were divided into TPVB group and non-TPVB group according to whether TPVB was performed preoperatively. The general data, perioperative variables, imaging detection and laboratory results of the patients were collected. After propensity score matching, the incidence of PPCs was compared between the two groups. Results After propensity score matching, there were 186 patients in each group. There were no significant differences in the baseline clinical data between the two groups(P>0.05). Compared with non-TPVB group, consumption of intraoperative opioids, the incidence rate of PPCs, proportion of rescue analgesia postoperatively and D-dimer level were significantly lower in the TPVB group(P<0.05). Conclusions Preoperative TPVB can provide effective analgesic effects and reduce the incidence of PPCs in the elderly patients undergoing thoracoscopic partial lung resection.

    Efficacy of camrelizumab plus chemotherapy for advanced esophageal squamous cell carcinoma in the elderly

    Objective To investigate the efficacy of programmed cell death protein 1(PD-1)inhibitor combined with paclitaxel and nedaplatin in the first line therapy for advanced esophageal squamous cell carcinoma(ESCC)in the elderly. Methods A total of 60 elderly patients with ESCC admitted to our hospital from January 2020 to January 2021 were enrolled and divided into the chemotherapy group(paclitaxel and nedaplatin) and the immunotherapy group(camrelizumab combined with paclitaxel and nedaplatin), with 30 cases in each group. The efficacy, T-lymphocyte subsets, and adverse reactions of the two groups were observed and compared. Results The disease control rate in the immunotherapy group was significantly higher than that in the chemotherapy group (83.3% vs 60.0%,P<0.05). There were no statistically significant differences in the levels of T-lymphocyte subsets between the two groups. The most common adverse reactions in immunotherapy group were anemia, reactive capillary hyperplasia and leukopenia, compared with anemia, leukopenia and vomit in chemotherapy group. Conclusions The combination of camrelizumab monotherapy with chemotherapy can improve the disease control rate with good safety in the elderly patients with advanced ESCC, which can be promoted in clinical practice.

    Correlation of serum ferritin level with diabetic foot in elderly patients with type 2 diabetes mellitus

    Objective To explore the correlation between serum ferritin (SF) levels and diabetic foot (DF) in the elderly patients with type 2 diabetes mellitus (T2DM). Methods The clinical data of 108 elderly T2DM patients admitted to Suzhou Municipal Hospital from August 2019 to April 2022 were analyzed retrospectively. All the patients were divided into DF group (n=48) and non-DF group (NDF, n=60). The correlation between SF level and DF risk was analyzed by curve fitting, and the cut-off value was defined by threshold effect analysis. Multiple Logistic regression analysis was used to analyze the risk factors for DF. Receiver operating characteristic (ROC) curve was used to evaluate the predictive performance of each indicator. Results When SF was>570 ng/mL, the risk of DF increased with the increase of SF (OR=0.975, 95%CI: 0.860-0.998, P<0.01). Glycosylated hemoglobin(HbA1c)>9.5%, total cholesterol(TC)>5.5 mmol/L, serum creatinine(Scr)>97.0 μmol/L, eGFR(estimated glomerular filtration rate)≤70.0 mL/(min·1.73 m2), SF>570.0 ng/mL and diabetic nephropathy(DN) were independent risk factors for DF (P<0.05). The ROC curve analysis showed that HbA1c, TC, Scr, eGFR, SF and DN were all highly effective indicators in predicting DF, of which SF showed the best efficiency. Conclusions As SF level increases, the risk of DF shows an upward trend. SF shows good predictive efficiency for the occurrence of DF. It is important to focus on SF in clinic to improve the patients' prognosis.

    Objective To explore the relationship of blood lipids levels with disease severity and prognosis in the elderly patients with sepsis. Methods A retrospective analysis was conducted on the clinical data of 162 elderly patients with sepsis admitted to Intensive Care Unit (ICU) of the Geriatric Hospital of Nanjing Medical University from January 2015 to January 2022. According to the 28-day mortality, the patients were divided into the survival group (n=104) and the death group (n=58). The clinical data and the levels of blood lipids of the two groups were compared. The correlation of blood lipids levels with inflammatory indicators and severity scores of the condition was analyzed. Receiver operating characteristic(ROC)curve of blood lipids and other clinical indicators in predicting the prognosis of the 28-day death was drawn. The patients were divided into 2 groups according to the cut-off value of the blood lipid index with the largest area under the curve (AUC), and the differences in prognosis were compared between the two groups. Results The levels of male ratio, body mass index (BMI), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C) in the death group were significantly lower than those in the survival group (P<0.05). The correlation analysis showed that the levels of TC,HDL-C,LDL-C were negatively correlated with Acute Physiology and Chronic Health Evaluation (APACHE Ⅱ)score and the level of procalcitonin(PCT)respectively, and HDL-C was negatively correlated with Sequential Organ Failure Assessmen (SOFA)score as well (P<0.05). ROC curve analysis showed that HDL-C had the highest predictive value (AUC=0.856) for the 28-day mortality, and the best cut-off value was 0.775 mmol/L. The 28-day mortality, incidence rate of multiple organ dysfunction, renal replacement therapy, and multidrug-resistant bacterial infections were significantly higher, and the time of mechanical ventilation and vasoactive drugs usage were longer in the low HDL-C group than those in the high HDL-C group (P<0.05). Conclusions The level of blood lipids, especially HDL-C, can well reflect the severity of elderly patients with sepsis, and it can be used as a good biomarker to predict the prognosis of elderly patients with sepsis.

    Correlation of NLR and PLR before immune consolidation therapy with prognosis in elderly patients with stage Ⅲ non-small cell lung cancer

    Objective To investigate the correlation of neutrophil to lymphocyte ratio(NLR), platelet to lymphocyte ratio (PLR) with the prognosis in the elderly patients with stage Ⅲ non-small cell lung cancer (NSCLC) treated with radical chemo radiotherapy (CRT) followed by immune consolidation therapy. Methods A total of 83 elderly patients with stage Ⅲ NSCLC admitted to Radiotherapy Department of Jiangsu Cancer Hospital who received radical CRT followed by programmed cell death protein (PD-1) inhibitor consolidation therapy were enrolled. The baseline data and hematological parameters of all patients were collected. X-tile software was used to select the best threshold values of NLR and PLR in the 83 patients. Progression-free survival (PFS) and overall survival (OS) were assessed by Kaplan-Meier. And log-rank test was used to compare OS and PFS between subgroups. The correlation of the baseline characteristics and hematological parameters with prognosis were assessed by univariate and multivariate Cox regression analysis. Results In this study, the median follow-up time was 16.3 months, and the median PFS was 15.2 months, and the median OS was 29.0 months. The best cut-off values of NLR and PLR predicting PFS and OS were 3 and 196 respectively. Subgroup analysis showed that low NLR and low PLR were associated with good PFS (HR=0.33, 95%CI:0.18-0.61, P<0.01; HR=0.32, 95%CI:0.18-0.58, P<0.01) and OS (HR=0.35, 95%CI:0.15-0.85, P<0.05; HR=0.22, 95%CI:0.09-0.54, P<0.01). Cox regression analysis showed PLR were associated with PFS, and ALC, were associated with OS. Conclusions Elderly patients with stage Ⅲ NSCLC who have undergone radical CRT are likely to benefit from subsequent immunotherapy. Low levels of NLR and PLR before PD-1 inhibitor therapy are associated with a better prognosis in the elderly patients, and PLR may be a prognostic indicator for PD-1 inhibitor consolidation therapy.

    Objective To investigate the predictive value of C-reactive protein to albumin ratio(CAR) combined with neutrophil to lymphocyte ratio(NLR) for postoperative cognitive dysfunction(POCD) in the elderly patients undergoing radical cystectomy for bladder cancer. Methods A total of 660 elderly patients who underwent radical cystectomy for bladder cancer in our hospital from January 2019 to October 2022 were enrolled in this study, and the patients were divided into the POCD group(n=182) and the non-POCD group(n=478) according to whether they presented with POCD. The basic clinical data of the patients were collected, and the levels of CAR and NLR were compared between the two groups. The influencing factors of POCD in the elderly patients undergoing radical cystectomy for bladder cancer were analyzed by Logistic regression model. Receiver operating characteristic(ROC) curve was used to analyze the predictive value of CAR, NLR and their combination for POCD in the elderly patients undergoing radical cystectomy for bladder cancer. Results There were significant difference in age, smoking, cerebrovascular disease, proportion of preoperative Mini Mental State Examination(MMSE) score <26 and operation time≥3 hours, and the levels of CAR and NLR between the POCD group and the non-POCD group(P<0.05 or P<0.01). Multivariate Logistic regression analysis showed that elevated CAR and NLR were independent risk factors for POCD in the elderly patients undergoing radical cystectomy for bladder cancer after adjusting other influencing factors. According to ROC curve analysis,the area under curve (AUC) of the combined detection of CAR and NLR in predicting POCD in the elderly patients undergoing radical cystectomy for bladder cancer was 0.894, which was significantly higher than that of CAR alone(AUC=0.787) or NLR alone(AUC=0.811), respectively, with statistically significant differences(P<0.01). Conclusions CAR combined with NLR has a high predictive value for POCD in the elderly patients undergoing radical cystectomy for bladder cancer.

    Blood pressure responses to moderate-intensity treadmill exercise test in elderly patients with hypertension

    Objective To explore the blood pressure(BP) responses to moderate-intensity treadmill exercise test(TET)in the elderly patients with hypertension, and to find out the cutoff value of BP in moderate-intensity exercise for diagnosis of hypertension. Methods A total of 208 patients who underwent TET in the cardiac rehabilitation clinic of the Jiangsu Province Official Hospital from March 2021 to September 2022 were selected and divided into hypertension group (112 cases) and normal BP group (96 cases). The levels of BP during TET in the two groups were recorded and compared. Multivariate Logistic regression was used to analyze the influencing factors of hypertension. Receiver operating characteristic(ROC)curve was drawn and the area under the curve(AUC) was calculated to analyze the moderate-intensity exercise BP value for diagnosis of hypertension. Results There were significant differences in the levels of BP and peak systolic blood pressure (SBP) at stage 1 and 2 of Bruce protocol in TET between hypertension group and normal BP group (P<0.05). Multivariate Logistic regression showed that the level of SBP at stage 2 of Bruce protocol was independently associated with hypertension. The ROC curve showed that the cut-off value of stage 2 SBP for diagnosis of hypertension was 168.5 mmHg, with a sensitivity of 86.4%,a specificity of 81.8%, an AUC of 0.876. Conclusions In the elderly hypertensive patients, the BP response in TET is significantly higher than that in the patients with normal BP. When the stage 2 SBP of Bruce protocol in TET is ≥ 168.5 mmHg, the possibility of hypertension in this elderly patient should be considered.

    Correlation between diabetic foot risk score and urinary microalbumin/creatinine ratio in elderly patients with type 2 diabetes

    Objective To analyze the correlation between diabetic foot risk score and urinary microalbumin/creatinine ratio (UMCR) in the elderly patients with type 2 diabetes mellitus (T2DM). Methods A total of 321 elderly patients with T2DM admitted to Department of Endocrinology of our hospital from January 2018 to December 2021 were enrolled in this study. According to the Gavin's foot risk factor weighted score, all patients were divided into the low risk foot group and the medium-high risk foot group. The general data and metabolic indexes of the patients were observed and compared. Multivariate Logistic regression analysis was used to analyze the influencing factors for medium-high risk foot, and receiver operating characteristic (ROC) curve was used to analyze the predictive value of UMCR for medium-high risk foot. Results There were significant differences in age, gender, living status, diabetes course, waist circumference, the levels of glycosylated hemoglobin (HbA1c), high-density lipoprotein cholesterol(HDL-C), ankle brachial index (ABI), serum uric acid(UA), UMCR, estimated glomerular filtration rate (eGFR), serum creatinine(Scr) and blood urine nitrogen(BUN) between the two groups (P<0.05). Multivariate Logistic regression analysis showed that UMCR was the risk factor for medium-high risk foot in the elderly T2DM patients(OR=1.002, 95%CI: 1.001-1.004, P<0.05). ROC curve showed that the area under the curve of UMCR in predicting medium-high risk foot was 0.739, the specificity was 78.6%, the sensitivity was 62.0% and the cut-off value was 43.23 mg/g. Conclusions UMCR is closely related to diabetic foot risk score, and has good predictive value for diabetic foot risk.

    Characteristics of cardiopulmonary function, exercise endurance, and the levels of NT-proBNP and TNT in elderly patients with stable chronic heart failure

    Objective To observe and analyze the characteristics of cardiopulmonary function and exercise endurance, the levels of N-terminal pro-brain natriuretic peptide (NT-proBNP) and Troponin T (TNT) in the elderly patients with stable chronic heart failure (CHF). Methods A total of 102 patients with stable CHF in our hospital from October 2020 to October 2022 were collected, and 50 healthy individuals who received physical examination during the same period were selected as control group. The cardiac function indexes, lung function indexes and serum levels of NT-proBNP, TNT and the 6-min walk distance (6MWD) were observed and compared among the groups. Results A total of 61 cases (59.80%) in the CHF group had lung function impairment, and the incidence rate of lung function impairment was higher in NYHA grade Ⅲ group than that in grade Ⅰ/Ⅱ group (69.23% vs 53.97%,P<0.05). The levels of left ventricular ejection fraction(LVEF), forced vital capacity(FVC), forced expiratory volume in one second(FEV1), FEV1/FVC, total lung capacity(TLC), diffusion capacity for carbon monoxide of the lung(DLCO) and 6MWD in CHF group were significantly lower than those in control group, and were significantly lower in NYHA grade Ⅲ group than those in grade Ⅰ/Ⅱ group. The levels of left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD), left atrial diameter (LAD) and blood NT-proBNP and TNT levels in CHF group were significantly higher than those in control group, and were significantly higher in NYHA grade Ⅲ group than those in grade Ⅰ/Ⅱ group (P<0.01). Pearson correlation analysis showed that the levels of LVEF and 6MWD in CHF patients were positively correlated with the levels of FVC, FEV1, FEV1/FVC, TLC and DLCO, while the levels of LVEDD, LVESD, LAD, NT-proBNP, TNT were negatively correlated with the levels of FVC, FEV1, FEV1/FVC, TLC and DLCO (P<0.05). Conclusions The elderly patients with stable CHF present with decline in cardiopulmonary function and exercise endurance, which are significantly correlated with the severity of cardiac dysfunction.

    Clinical application of improved frailty index to predict complications after radical prostatectomy assisted by robot in elderly patients

    Objective To evaluate the application of modified frailty index(mFI) in predicting the complications after robot-assisted radical prostatectomy in the elderly patients. Methods A retrospective analysis was performed on the clinical data of 100 patients who underwent robot-assisted radical prostatectomy in the Second Affiliated Hospital of Xi 'an Jiaotong University from June 2019 to October 2022. According to mFI score, the patients were divided into the frailty group (mFI≥0.27) and the non-frailty group (mFI<0.27).The differences in general clinical data, mFI score and the incidence of postoperative complications between the two groups were analyzed. Multivariate Logistic regression analysis was performed to explore the risk factors of postoperative complications. Results A total of 100 patients meeting the criteria were enrolled in this study, including 37 cases in the frailty group and 63 cases in the non-frailty group. There were statistically significant differences in American Society of Anesthesiologists(ASA) grading and age between the two groups (P<0.05). The incidence rates of the complications in cardiovascular system, respiratory system and nervous system in frailty group were significantly higher than those in the non-frailty group (P<0.01). Multivariate Logistic regression analysis showed that the frailty was the independent risk factors for the complications in cardiovascular system (OR=7.344, 95%CI:1.196-45.093, P=0.031), respiratory system (OR=5.849, 95%CI: 1.668-20.507, P=0.006) and nervous system (OR=5.521, 95%CI:1.686-18.083, P=0.005). Conclusions mFI score can be used as an effective tool to evaluate the frailty state of the elderly patients undergoing robot-assisted radical prostatectomy. The patients with mFI≥0.27 have an increased risk of postoperative multisystem complications.

    Analysis of status and influencing factors of nursing dependence in home-based patients with Parkinson's disease

    Objective To explore the status of nursing dependence in home-based patients with Parkinson's disease(PD) and analyze the influencing factors to provide a reference for formulating targeted intervention measures. Methods A total of home-based 172 patients with PD from March 2021 to July 2022 after treatment at Xiangyang First People's Hospital were enrolled by convenience sampling. Self-designed general data survey scale, Care Dependency Scale (CDC), Hamilton Depression Scale (HAMD-17) and Montreal Cognitive Assessment (MoCA) were surveyed. Multivariate Logistic regression was used to analyze the influencing factors of care dependence in home-based patients with PD. Results The total score of nursing dependence of home-based patients with PD was 61.61±11.11, and the incidence of nursing dependence was 61.0%. Logistic regression analysis showed that the disease course, postural instability/gait disorder, depression and cognitive dysfunction were the influencing factors of nursing dependence in home-based patients with PD (P<0.05). Conclusions The home-based patients with PD have high level of nursing dependence. Attention should be paid to the patients with long course, postural instability/gait disorder, depression and cognitive dysfunction. The effective intervention measures should be taken to improve the quality of life of the patients.

    Objective To explore the application of project management method based on multidisciplinary collaboration in the nutrition management in the elderly patients with dysphagia. Methods A total of 168 elderly patients with dysphagia in our hospital from March 2021 to March 2022 were randomly divided into the intervention group and the control group, with 84 cases in each group. The intervention group received nutrition management by the project management method based on multidisciplinary cooperation, and the control group received traditional nutrition management. After 3 months of intervention, the incidence of dysphagia-related complications, nutritional status and swallowing quality of life were observed and compared between the two groups. Results The incidence rate of dysphagia-related complications in the intervention group was significantly lower than that of the control group(4.8% vs 20.2%,P<0.05).After 3 months of intervention, the scores of the Chinese version of the elderly nutrition scale and the levels of the nutrition-related laboratory indexes were more significantly improved than those before intervention in the two groups, especially in the intervention group(P<0.05).The score of swallowing-quality of life was significantly higher than that before intervention in the two groups, especially in the intervention group (P<0.01). Conclusions The project management method based on multidisciplinary cooperation is helpful to improve the nutritional indicators and swallowing quality of life in the elderly patients with dysphagia.
