负荷深呼吸康复训练对老年慢性阻塞性肺疾病缓解期病人心肺功能及生活质量的影响

Effect of load deep breathing rehabilitation training on cardiopulmonary function and quality of life in elderly patients with chronic obstructive pulmonary disease at remission stage

摘要: 目的 观察负荷深呼吸康复训练对老年COPD缓解期病人心肺功能及生活质量的影响。 方法 选择2017年7月至2020年6月本院收治的82例老年COPD缓解期病人作为研究对象,采用随机数表法将其分为观察组和对照组,每组各41例。对照组进行常规治疗,观察组在对照组的基础上联合进行负荷深呼吸康复训练。比较2组治疗前后肺功能指标[FVC、FEV1、FEV1/FVC、血氧饱和度(SaO2)]、心功能指标[每搏输出量(SV)、LVEF、左心室舒张末内径(LVEDD)、左心室收缩末内径(LVESD)]和圣乔治呼吸问卷(SGRQ)评分、6分钟步行试验(6MWT)结果。 结果 与对照组相比,观察组病人治疗3个月后FVC、FEV1、FEV1/FVC、SaO2、SV、LVEF水平较高,LVEDD、LVESD水平较低,差异均有统计学意义(P<0.05)。观察组治疗后1、2、3、4个月SGRQ评分均低于同期对照组,且随时间增长呈下降趋势,差异均有统计学意义(P<0.05)。观察组治疗3个月后6 min步行距离明显远于对照组(P<0.05)。 结论 负荷深呼吸康复训练对老年COPD缓解期病人具有改善心肺功能的价值,并能提升病人的生活质量及运动表现,值得临床推广。

Abstract: Objective To investigate the effect of load deep breathing rehabilitation training on cardiopulmonary function and quality of life in the elderly patients with chronic obstructive pulmonary disease (COPD) at remission stage. Methods A total of 82 elderly patients with COPD at remission stage from July 2017 to June 2020 in our hospital were selected, and they were divided into observation group and control group by random number table method, with 41 patients in each group. The control group received routine treatment, and the observation group was given load deep breathing rehabilitation training on the basis of the control group. The pulmonary function indexes [forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC, oxygen saturation (SaO2)], cardiac function indexes [stroke volume (SV), left ventricular ejection fraction (LVEF), left ventricular end-diastolic diameter (LVEDD), left ventricular end-systolic diameter (LVESD)], St. George's Respiratory Questionnaire (SGRQ) scores and the result of 6-minute walk test (6MWT) were compared between the two groups before and after treatment. Results Compared with the control group, the levels of FVC, FEV1, FEV1/FVC, SaO2, SV and LVEF were higher, and the levels of LVEDD and LVESD were lower in the observation group after 3 months of treatment (P<0.05). Compared with the control group, the SGRQ scores of the observation group 1, 2, 3 and 4 months after treatment were significantly decreased (P<0.05) at a time-dependent manner. After 3 months of treatment, the 6 minutes walk distance of the observation group was significantly longer than that of the control group (P<0.05). Conclusions Load deep breathing rehabilitation training shows a value in improving the cardiopulmonary function in the elderly patients with COPD at remission stage, which could improve the quality of life and the exercise performance of the patients.
