    Effect of preemptive analgesia with butorphanol tartrate combined with dexmedetomidine on emergence agitation and postoperative cognitive dysfunction in elderly patients with bladder cancer undergoing radical operation

    Objective To investigate the effect of butorphanol tartrate combined with dexmedetomidine preemptive analgesia on emergence agitation and postoperative cognitive dysfunction (POCD) in the elderly patients with bladder cancer undergoing radical operation. Methods A total of 90 elderly patients who underwent radical resection of bladder cancer in Xuzhou Central Hospital from August 2019 to December 2021 were enrolled in this study. The patients were randomly divided into control group and observation group, with 45 cases in each group. The control group received dexmedetomidine preemptive analgesia, and the observation group received butorphanol tartrate combined with dexmedetomidine preemptive analgesia. The levels of hemodynamic indexes, agitation score, the scores of Mini Mental State Examination(MMSE), visual analogue scale (VAS) and the levels of inflammatory factors at different time points and the incidence rate of POCD were compared between the two groups. Results The levels of mean arterial pressure and heart rate in the control group at extubation immediately were significantly higher than those in the observation group and those before anesthesia (P<0.05). The incidence rate of emergence agitation in the observation group was 8.89%, compared with 28.89% in the control group(P<0.05). MMSE scores in the observation group were significantly higher than those in the control group 1 and 3 days after operation(P<0.05). There were 4 cases of POCD(8.89%) in the observation group and 15 cases of POCD(33.33%) in the control group after operation, with significant difference(P<0.05). The VAS score in the observation group was significantly lower than that in the control group 4 h, 12 h and 1 d after operation(P<0.05). Compared with the control group, the levels of IL-1β, IL-6, TNF-α in the observation group 12 h, 1 d and 3 d after operation were significantly decreased(P<0.05). Conclusions For the elderly patients with bladder cancer undergoing radical operation, butorphanol tartrate combined with dexmedetomidine preemptive analgesia can keep the extubation hemodynamics more stable, and can effectively reduce the postoperative pain, the expression of inflammatory factors and the incidence of emergence agitation and POCD.

    Objective To investigate the therapeutic effect of Mulligan technique combined with extracorporeal shock wave therapy in the treatment of scapulohumeral periarthritis in the elderly. Methods From January 2019 to July 2022, a total of 50 elderly patients diagnosed as scapulohumeral periarthritis in Rehabilitation Medical Center of the First Affiliated Hospital of Nanjing Medical University were selected and divided into the control group and the research group according to the therapy, with 25 cases in each group. The research group received Mulligan technique combined with extracorporeal shock wave, while the control group received ordinary joint mobilization combined with extracorporeal shock wave. Both groups were treated 3 times a week for 6 consecutive weeks. The shoulder pain scale (visual analogue scale, VAS) and shoulder function scale (Constant-Murley) were used to evaluate the efficacy before and after treatment. Results The VAS score and Constant-Murley score in both groups were significantly improved after treatment compared with those before treatment (P<0.001), especially in the research group (P<0.001). Conclusions Mulligan technique combined with extracorporeal shock wave therapy is effective in the treatment of elderly patients with scapulohumeral periarthritis, which has the value of clinical application and promotion.

    Prevalence and influencing factors of sarcopenia in community elderly

    Objective To investigate the prevalence and influencing factors of sarcopenia among the community elderly in Anhui Province, so as to provide the reference for community primary medical staffs to identify, prevent and control sarcopenia. Methods From February 2019 to December 2021, 1716 elderly people in the communities of Hefei, Bengbu and Chuzhou who signed contracts with family doctors were enrolled as the study subjects by using the convenient sampling method. According to 2019 consensus report of the Asian Working Group for Sarcopenia, the subjects were screened for sarcopenia and underwent relevant health examinations. Results The prevalence rate of sarcopenia among the community elderly in Anhui Province was 12.47% (214/1716). There were significant differences in gender, age, body mass index (BMI), waist-to-hip ratio, hypertension, dyslipidemia, joint pain, sufficient physical exercise, vital capacity and choice reaction time between the elderly with or without sarcopenia (P<0.05). Multiple Logistic regression analysis showed that advanced age and joint pain were risk factors for sarcopenia, and higher BMI, sufficient physical exercise and higher vital capacity were protective factors for sarcopenia. Conclusions The prevalence rate of sarcopenia among the community elderly in Anhui Province is high. Community primary medical staffs should guide older people with joint pain to increase exercise to maintain higher vital capacity, and increase nutrition intake to prevent and control sarcopenia.

    Association analysis with stratification between depression and adverse childhood experiences in middle-aged and elderly Chinese

    Objective To investigate the association between characteristic-specific adverse childhood experiences (ACEs) events and depression among middle-aged and elderly Chinese. Methods The study subjects were screened from the China Health and Retirement Longitudinal Study (CHARLS) database released by Peking University, based on research objective and inclusion/exclusion criteria. The median interpolation and multiple interpolation methods were adopted for the missing data, and multivariate analysis with stratification and Logistic regression model was adopted to estimate the effect of ACEs on depression in the subjects with different genders, ages, education levels and income. Poisson regression model was used to analyze the sensitivity of Logistic regression model. Results A total of 13 561 samples from CHARLS were enrolled in this study. The prevalence of depression was 11.03%, and 94.17% of the subjects had at least one ACE. Logistic regression analysis with stratification showed that there was statistically significant association between depression and ACEs in all subgroups (P<0.05), with relatively higher OR in men (OR=1.373), population aged 60 years and older (OR=1.327), with primary school education and below (OR=1.325), and low income (OR=1.310). The results of Poisson regression model showed that Logistic regression model with stratification was statistically robust. Conclusions The risk of depression after ACEs is relatively higher in men, the elderly, or the population with lower level of education and income. The preliminary understanding of these high-risk characteristics will promote healthy aging.

    Relationship between decline of intrinsic capacity and frailty in elderly inpatients

    Objective To explore the relationship between decline of intrinsic capacity (IC) and frailty syndrome in the elderly inpatients. Methods A total of 370 elderly inpatients in our hospital from January 2019 to November 2021 were enrolled, and the FRAIL scale and the intrinsic capacity decline assessment tool were used to evaluate frailty and IC decline of the patients respectively. Multivariate ordinal Logistic regression was used to analyze the influencing factors of frailty and the influence of different dimensions of IC decline on frailty. Results Among the 370 elderly hospitalized patients, 119 (32.2%) presented with non-frailty, 156 (42.2%) with pre-frailty, and 95 (25.7%) with frailty. The score of IC was 4(3, 5), and the patients with decline in activity, nutrition, cognition, vision, hearing and psychology dimension accounted for 67.3%, 23.2%, 25.4%, 58.1%, 35.9% and 12.2%, respectively. Multivariate ordinal Logistic regression analysis indicated that exercise habits, basic activities of daily living (BADL) score, instrumental activities of daily living (IADL) score and the degree of IC decline were the influencing factors of frailty in the elderly inpatients. Depression had the greatest impact on frailty among the six dimensions of IC. Conclusions The decline of IC is common in the elderly inpatients, and the elderly with IC decline are prone to frailty.

    Comparison of multi-balloon cushion and manual reduction combined with vertebroplasty in the treatment of elderly patients with osteoporotic vertebral compression fractures

    Objective To compare the clinical efficacy of multi-balloon cushion and manual reduction combined with vertebroplasty in the treatment of the elderly patients with osteoporotic vertebral compression fractures (OVCF). Methods A total of 217 elderly patients with OVCF admitted in our hospital from January 2019 to February 2021 were divided into the adjustable multi-balloon cushion group (group A, n=48) and manual reduction combined with vertebroplasty group (group B, n=169). The visual analogue scale(VAS) score, Oswestry disability index (ODI), anterior vertebral body compression rate and recovery percentage were compared between the two groups before and after treatment. Results There were no statistical significances in the basic data, VAS score, ODI score and compression rate of anterior vertebral body between the two groups before treatment (P>0.05). The VAS score and ODI score in the two groups three months after treatment were all lower than those before treatment, especially in group B (P<0.05). The recovery rate of anterior vertebral body height in group B was higher than that in group A (P<0.05). In group A, lower extremity deep vein thrombosis occurred in 1 case, and no fracture nonunion and refracture were observed. The refracture rate in group B was 1.78%, and the bone cement leakage rate was 26.63%. Conclusions For elderly OVCF patients, the multi-balloon cushion is an effective, safe and reliable conservative method, but manual reduction combined with vertebroplasty is more effective, which is need to pay more attention to the complications, such as bone cement leakage.

    Objective To investigate the effect of intensive rosuvastatin treatment on the inflamm-aging and the efficacy in the elderly patients with acute coronary syndrome (ACS) receiving percutaneacs coronary intervention(PCI). Methods A total of 128 elderly ACS patients admitted to our hospital from June 2018 to June 2020 were randomly divided into control group and observation group, with 64 cases in each group. Both groups were treated with PCI in emergency, and rosuvastatin 10 mg was given to the control group and intensive rosuvastatin 20 mg was given to the observation group. The levels of vascular endothelial growth factor (VEGF), vascular cell adhesion molecule-1 (VCAM-1) and pigment epithelium derived factor (PEDF) were detected and compared between the two groups. The size and thickness of plaque, intima-media thickness (IMT) and calcification score were compared between the two groups. After 12 months of follow-up, the major adverse cardiovascular events (MACE) in the two groups were observed. Results There were no significant differences in the levels of VEGF, VCAM-1 and PEDF between the two groups before PCI. After PCI, the levels of VEGF and VCAM-1 in both groups decreased significantly, especially in the observation group (P<0.05); The level of PEDF increased significantly in the observation group (P<0.05), but there was no significant change in the control group (P>0.05). The size and thickness of plaque, IMT and plaque calcification score in both groups decreased significantly after PCI, especially in the observation group (P<0.05). After 12 months of follow-up, the incidence rate of MACE in the observation group was significantly lower than that in the control group (4.69% vs 20.31%, P<0.05). Conclusions Intensive rosuvastatin treatment can effectively reduce the levels of VEGF and VCAM-1, improve the level of PEDF, increase the stability of atherosclerotic plaque, and reduce the carotid atherosclerosis in the elderly patients with ACS, which is of great significance for the prevention, treatment and prognosis of ACS.

    Comparison of characteristics of periodontal health between elderly patients with type 2 diabetes mellitus and healthy people

    Objective To compare the characteristics of periodontal health between elderly patients with type 2 diabetes mellitus and elderly people receiving physical examination. Methods A cross-sectional study of periodontal health was conducted in the elderly patients with type 2 diabetes mellitus (diabetes group, 70 cases) and elderly people receiving physical examination (control group, 100 cases). The bleeding on probing (BOP), the depth of maximum periodontal probing depth (PD), the losing teeth due to periodontal disease, the stage and grade of periodontitis were compared between the two groups. Results The positive rate of BOP, the detection rate of PD=3-5 mm or >5 mm, and the percentage of teeth lost due to periodontal disease in diabetes group were significantly higher than those in control group (P<0.001). The number of BOP tooth surface, the number of teeth with PD=3-5 mm or >5 mm and the number of lost teeth due to periodontal disease in diabetes group were higher than those in control group (P<0.001). The prevalence rate of periodontitis in diabetes group was significantly higher than that in control group (97.1% vs 45.0%, P<0.001). The proportion of periodontitis in stages Ⅲ and Ⅳ according to severity, the proportion in Grade C according to the progression speed of periodontitis and the proportion in extensive type according to the range of affected teeth were significantly higher in diabetes group than those in control group(P<0.05). Conclusions The periodontal health of the elderly patients with type 2 diabetes mellitus is not optimistic, with high detection rate, serious degree, fast progression speed and extensive range. Therefore, the elderly patients with diabetes should be screened for periodontal disease routinely and receive periodontal sequence therapy timely.

    Clinical predictive value of NLR combined with MPV on prognosis of intravenous thrombolysis in elderly patients with acute ischemic stroke

    Objective To explore the clinical predictive value of neutrophil-lymphocyte ratio (NLR) combined with mean platelet volume (MPV) on prognosis of intravenous thrombolysis in the elderly patients with acute ischemic stroke (AIS). Methods A total of 76 elderly patients with AIS who received intravenous thrombolysis in our hospital from January 2017 to June 2020 were selected and divided into good prognosis group(≤2 points) and poor prognosis group (>2 points) according to Modified Rankin Scale (MRS). The general information and clinical data of the patients were collected, and the levels of white blood cell count (WBC), neutrophil count (N), lymphocyte count (L), NLR, platelet count-lymphocyte count ratio (PLR), platelet count (PLT), MPV, platelet distribution width (PDW), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), homocysteine (Hcy), total cholesterol (TC) and creatinine were detected and compared between the two groups. Multiple Logistic regression analysis was used to analyze the risk factors for poor prognosis. Receiver operating characteristic (ROC) curve was used to evaluate the effect of NLR combined with MPV on the prognosis of intravenous thrombolysis in the elderly patients with AIS. Results The levels of WBC, NLR, PLR, MPV and National Institute of Health Stroke Scale (NIHSS) score in the poor prognosis group were significantly higher than those in the good prognosis group (P<0.05). Multiple Logistic regression analysis showed that NLR and MPV were the independent influencing factors of poor prognosis (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of NLR combined with MPV was 0.845 (95%CI: 0.798-0.957), with a sensitivity of 88%, and a specificity of 91%. Conclusions NLR combined with MPV has a high predictive value for the prognosis of intravenous thrombolysis in the elderly patients with AIS.

    Analysis of clinical features of sepsis-associated hemophagocytic lymphohistiocytosis in the elderly

    Objective To analyze the clinical characteristics, laboratory test characteristics and prognosis of the elderly patients with sepsis-related hemophagocyic lymphohistiocytosis (HLH), and to provide evidence for early detection and reduction of mortality. Methods The clinical information of 32 elderly patients with sepsis-related HLH (aged group) and 24 non-elderly patients with sepsis-related HLH (non-aged group) from June 2013 to March 2022 in Nanjing First Hospital were retrospectively collected. The clinical characteristics and mortality of the two groups were analyzed and compared. Results The proportion of the patients with hypertension and cerebrovascular diseases was significantly higher and the survival time was shorter in the aged group than that in the non-aged group. The 28-day mortality rate and 90-day mortality rate in the aged group were higher than those in the non-aged group (62.5% vs 25.0%, 81.3% vs 45.8%, P<0.05), and there were significant differences in the levels of blood urine nitrogen (BUN), serum creatinine (SCr), Cystatin C (CysC) and Troponin T (TnT ) between the two groups (P<0.05). The aged group had a higher incidence rate of hepatosplenomegaly compared with the non-aged group (66.7% vs 25.0%, P<0.05). There was no significant difference in mortality due to different treatment regimens containing hormones between the two groups (P>0.05). Conclusions Sepsis-related HLH in the elderly patients has a higher mortality rate and is prone to causing heart and renal damage. Early detection and early treatment are crucial to reduce the mortality rate of the patients.

    Construction and validation of a nomogram prediction model for new fractures after PVP in patients with osteoporotic thoracolumbar fractures

    Objective To explore the influencing factors of new fractures after percutaneous vertebroplasty (PVP) in the patients with osteoporotic thoracolumbar fractures (OTF), and to establish a nomogram early warning model and verify the predictive performance of the model. Methods A total of 193 patients with OTF who underwent PVP in our hospital from January 2017 to December 2018 were retrospectively selected as the modeling group, and 63 patients with OTF who underwent PVP from January to December 2019 were selected as the validation group. The end point of follow-up was 2 years after surgery or the time of diagnosis of new postoperative fractures. The incidence of postoperative fractures in the modeling group was observed, and the independent risk factors for new postoperative fractures were screened by univariate and multivariate Cox proportional hazards models. R software was used to establish a nomogram model to predict the risk of new fractures after surgery, and the predictive effect of the model was verified internally and externally. Results The incidence rate of new fractures after PVP was 21.76% (42/193). Multivariate Cox regression analysis showed that lumbar spine T value, intravertebral fissure-like degeneration, height recovery rate of injured vertebral anterior edge, bone cement injection volume, and bone cement leakage were the independent risk factors for new fractures after PVP in the patients with OTF. Based on the multivariate Cox regression analysis results, a nomogram model for predicting new postoperative fractures was established. The C-index of the prediction model was 0.730 (95%CI: 0.706-0.784) in the validation group and 0.796 (95%CI: 0.724-0.836) in the modeling group, and the areas under the receiver operating characteristic curve for predicting the incidence of new fractures at 1 year and 2 years after surgery were 0.778, 0.721 in the validation group and 0.820, 0.772 in the modeling group, respectively. The slopes of the calibration curves of the two groups were both close to 1, and the threshold probability of the decision curve analysis (DCA) was 0.05-1, and the model showed a positive net benefit. Conclusions This study establishes a nomogram prediction model based on five independent risk factors of new fractures after PVP for OTF, which has a good predictive performance and can provide a certain reference value for clinical medical staff to prevent fractures after PVP.

    Effects of uric acid on the expression of GLUT9 and URAT1 of cartilage in elderly patients with knee osteoarthritis

    Objective To investigate the correlation between serum uric acid (UA)level and the expression of UA transporter protein of cartilage tissue in the elderly patients with knee osteoarthritis(KOA). Methods A totoal of 30 elderly patients with KOA in the Department of Orthopedics, Hebei General Hospital from January 2018 to December 2019 were enrolled. According to the median level of UA, the patients were divided into UA<254.6 μmol/L group (group A) and UA≥254.6 μmol/L group (group B). The expression levels of glucose transporter 9 polypeptide (GLUT9) and urate transporter 1(URAT1) in cartilage tissue in the two groups were detected and compared. Results The levels of weight and body mass index(BMI), and visual analogue scale (VAS) score in group A were significantly lower than those in group B (P<0.05). Pearson analysis showed that VAS score was positively correlated with UA level(r=0.396,P<0.05). Compared with group A, the expression levels of GLUT9 and URAT1 mRNA and protein were significantly increased in group B (P<0.05). Conclusions The increase of UA level may increase the expression of GLUT9 and URAT1 protein in cartilage and the level of UA in chondrocytes, which induces the oxidative damage of chondrocytes, thereby participating in the pathogenesis of KOA.

    Correlation between skeletal muscle index and triglyceride/high density lipoprotein cholesterol ratio in community-dewlling elderly adults

    Objective To investigate the correlation between appendicular skeletal muscle index (ASMI) and triglyceride/high density lipoprotein cholesterol ratio (TG/HDL-C) in the community-dewlling elderly adults. Methods A total of 192 elderly subjects (aged ≥65 years old) who met the inclusion criteria and completed relevant clinical data were enrolled in this study. The baseline data and the levels of hematological indicators were collected. According to ASMI measured by the body composition analyzer on the day of physical examination, all the subjects were divided into the low quantile group (ASMI < 8.41), middle quantile group (ASMI=8.41-9.16) and high quantile group (ASMI> 9.16), with 64 cases in each group. The baseline data and the levels of biochemical and body composition indicators were compared among the three groups. Spearman correlation analysis was used to analyze the correlations between ASMI and related variables. Multivariate Logistic regression was used to analyze the influencing factors of ASMI. Results Among the elderly in the community, with the increase of ASMI, TG/HDL-C also increased significantly in the three groups (P<0.05). Spearman correlation analysis showed that ASMI was positively correlated with gender, systolic blood pressure (SBP), diastolic blood pressure (DBP), basal metabolic volume, muscle mass, waist-to-hip ratio (WHR), TG/HDL-C and alanine aminotransferase (ALT). Multivariate Logistic regression analysis showed that gender (OR=0.130, 95%CI: 0.027-0.635), TG/HDL-C (OR=0.575, 95%CI: 0.332-0.996), muscle mass (OR=0.609, 95%CI: 0.485-0.766) and basal metabolism volume (OR=1.011, 95%CI: 1.001-1.021) were the independent influencing factors of ASMI. Conclusions TG/HDL-C is helpful to predict the declining trend of ASMI in the elderly, and has clinical significance for screening elderly patients with suspected sarcopenia in the community.

    Analysis of characteristics and influencing factors of health literacy in elderly patients with chronic obstructive pulmonary disease in Ningxia

    Objective To investigate the characteristics of health literacy of the elderly patients with chronic obstructive pulmonary disease (COPD) in Ningxia and analyze the influencing factors. Methods Using convenience sampling method, the elderly COPD patients hospitalized in Respiratory and Critical Care Medical Department of three tertiary hospital in Ningxia from November 2020 to June 2021 were enrolled in this study. The general situation questionnaire, modified Medical Research Council dyspnea scale (mMRC), COPD knowledge questionnaire (COPD-Q), 15-iterm Geriatric Depression Scale(GDS-15), Chinese version of housebound scale and Activity of Daily Living (ADL) scale were used for investigation. A total of 435 valid questionnaires were collected. Single factor analysis and multiple linear stepwise regression were used to analyze the influencing factors of health literacy in the elderly patients with COPD. Results The average COPD-Q score of 435 elderly patients with COPD was 5.89±2.26. Multivariate linear stepwise regression analysis showed that gender, primary caregiver, medical payment, oxygen therapy, smoking, respiratory muscle training and ADL score were the independent influencing factors for health literacy in the elderly COPD patients (all P<0.05). Conclusions The health literacy in the elderly patients with COPD is at a low level, which is affected by gender, primary caregiver, medical payment method, oxygen therapy, smoking, respiratory muscle training and ADL score. Medical staff should take personalized intervention measures to improve patients’ ability to solve their own health problems according to their individual conditions.

    Effect of Liu Zi Jue exercise combined with elastic belt resistance training on pulmonary rehabilitation in elderly patients with chronic obstructive pulmonary disease at stable stage

    Objective To investigate the effect of Liu Zi Jue exercise combined with elastic belt resistance training on pulmonary function and quality of life in the elderly patients with chronic obstructive pulmonary disease (COPD) at stable stage. Methods A total of 60 elderly COPD patients at stable stage admitted to Department of Geriatrics and Department of Respiratory Medicine of Affiliated Hospital of Nanjing University of Chinese Medicine from June 2019 to June 2021 were selected. According to the random numbers generated by computer software, all the patients were randomly divided into the control group and the experimental group, with 30 cases in each group. The control group received elastic belt resistance training on the basis of standardized treatment, and the experimental group received Liu Zi Jue breathing exercise on the basis of the control group. Both groups were trained 5 times a week (30 min each time) for 6 months. The pulmonary function [forced expiratory volume in 1 second (FEV1), forced vital capacity (FVC) and FEV1/FVC], 6-min walking distance (6MWD), and Chronic Obstructive Pulmonary Disease Assessment Test (CAT) scores of the patients were evaluated before the intervention and after 3, 6 months of the intervention, respectively. Results After 3 months of the intervention, the levels of FEV1, FVC, FEV1/FVC and 6MWD, and the score of CAT in both groups were significantly improved (P<0.05), and the levels of FEV1, FVC and FEV1/FVC, and the score of CAT showed significant differences between the two groups (P<0.05). After 6 months of the intervention, the levels of FEV1, FVC, FEV1/FVC and 6MWD, and the score of CAT in both groups were significantly imporved compared with those after 3 months of the intervention, and differences between the two groups were significant (P<0.05). Conclusions Liu Zi Jue exercise combined with elastic belt resistance training can effectively increase exercise endurance, improve pulmonary function and quality of life of the elderly COPD patients at stable stage.
