    Changes of gut microbiota in aged mice with chronic neuropathic pain-related anxiety

    Objective To observe the anxiety-like behavior and the change of gut microbiota in aged mice with chronic neuropathic pain-related anxiety. Methods The male C57BL/6 mice aged 16 months were randomly divided into model group and control group, with 6 mice in each group. Spared nerve injury (SNI) surgery was performed in the sciatic nerve to establish neuropathic pain-related anxiety model. On the 21st day after surgery, Von-Frey hairs test was used to detect mechanical hyperalgesia. The open field test (OFT) and elevated plus maze (EPM) experiments were used to detect anxiety-like behaviors. 16S rDNA sequencing was used to analyze the gut microbiota. Results The mice in the model group showed decreasing mechanical thresholds in the Von Frey test (P<0.001). On the 21st day after SNI, the activity time of the mice entering the central area of OFT and the open arm area of EPM experiments were significantly less than those of the control group(P<0.05), suggesting that SNI mice had anxiety-like behaviors. The total distance of the model group and the control group in OFT and EPM experiments showed no significant difference. The analysis of gut microbiota showed that the gut microbiota structure in the SNI mice was disordered and the diversity of the flora was reduced. In the SNI mice, 2β-proteobacteria were significantly reduced at the level of phylum classification, Lactobacillus was significantly reduced at the order level, and Prevotella was significantly reduced at the family level. Conclusions Aged mice with neuropathic pain-related anxiety have anxiety-like behaviors and show significant changes in gut microbiota.

    Morphological changes of diaphragm and related factors in elderly patients with AECOPD

    Objective To analyze the morphological changes of diaphragm and the related factors in the elderly patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods A total of 58 elderly AECOPD inpatients admitted to Shaanxi Provincial People's Hospital were collected as the AECOPD group, and 49 healthy subjects were collected as the control group. The diaphragm thickness at function residual capacity(TdiFRC), diaphragm thickness at forced vital capacity(TdiFVC), diaphragm thickness at residual volume(TdiRV), diaphragmatic displacement during tidal breathing (DD) and diaphragmatic displacement during forced breathing (DDmax) were measured by using Ultrasonography, then the thickening fraction (TF) was calculated. All the indicators between the two groups were compared and multiple linear regression was used to analyze the influencing factors of TF and DD. Results The AECOPD patients were divided into two groups according to GOLD classification: mild-moderate group and severe-very severe group. Compared with the control group and the mild-moderate group, the severe-very severe group showed significantly difference in TdiFRC, TdiRV, DD, DDmax and TF values (all P<0.05);And mild-moderate group showed lower level in TdiFVC,DDmax and TF than those in the control group ( all P<0.05). TF was positively correlated with forced expiratory volume in 1 second percentage predicted (FEV1%pred) (β=0.547, P<0.05), and negtively correlated with the duration of COPD (β=-0.959, P<0.05) and the number of acute exacerbations in the past 1 year (β=-5.978, P<0.05), while DD was lowly negatively correlated with the duration of COPD (β=-0.027, P<0.05) and the number of acute exacerbations within 1 year (β=-0.150, P<0.05). Conclusions Both TF and DDmax in the elderly patients with AECOPD decrease significantly, which are mainly affected by the disease itself, and it is necessary to actively control the progression of COPD and reduce the frequency of exacerbation.

    Meta-analysis of the effect of resistance training on inflammation in elderly people

    Objective To investigate the effect of resistance training on inflammation in the elderly and to provide some basis for the formulation of exercise prescription. Methods Randomized controlled trials on the effects of resistance training on inflammation factors in the elderly people were retrieved from the databases.The retrieved literatures were screened and the data were extracted, and Meta-analysis was performed using RevMan 5.3 software. Results A total of 11 literatures were enrolled, including 404 subjects.The results showed that resistance training reduced the levels of inflammation factors in the elderly people, including C-reactive protein (CRP), tumor necrosis factor-α(TNF-α) and interleukin-6(IL-6). Subgroup analysis showed that the level of CRP was decreased more significantly when the number of exercise was over 8. The effect of high intensity resistance training on TNF-α was better in elderly people. Resistance training was more effective in reducing IL-6 when the duration was less than 12 weeks, the number of exercise was over 8, and the frequency was more than 2 times per week. Conclusions Resistance training has a good effect on alleviating chronic inflammation in the elderly.

    Effect of plasma exchange plus double plasma molecular adsorption system combined with night nutrition intervention in elderly patients with liver failure

    Objective To investigate the effect of plasma exchange (PE) + double plasma molecular adsorption system (DPMAS) combined with night nutrition intervention in the elderly patients with liver failure. Methods A total of 72 elderly patients with liver failure treated in the First People's Hospital of Lianyungang from June 2019 to December 2021 were selected as the research objects. They were divided into control group (n=36) and observation group (n=36) randomly. The patients in the control group were treated with PE+DPMAS on the basis of routine comprehensive medical treatment, and the observation group received night nutrition intervention additionally on the basis of the control group. The Child Pugh score, nutritional risk screening (NRS-2002)score and blood nutritional indexes were observed and compared between the two groups. Results The Child Pugh score, NRS-2002 score, serum levels of albumin, prealbumin and transferrin in the two groups were significantly improved 1 week and 1 month after intervention (P<0.05), and there were significant differences between the two groups 1 month after the intervention (P<0.05). Conclusions PE+DPMAS combined with night nutrition intervention has obvious therapeutic effect on the elderly patients with liver failure, which can reduce the risk of malnutrition and improve the liver reserve function of the patients.

    Predictive value of serum ICAM-1, LPS, and sPD-L1 for infectious pancreatic necrosis secondary to severe acute pancreatitis in elderly patients

    Objective To investigate the predictive value of serum intercellular adhesion molecule-1 (ICAM-1), lipase (LPS), and soluble programmed death ligand 1 (sPD-L1) for infectious pancreatic necrosis (IPN) secondary to severe acute pancreatitis (SAP) in the elderly. Methods Forty-five elderly patients with SAP admitted to our hospital from January 2019 to July 2021 were selected and divided into observation group (17 cases) and control group (28 cases) according to whether they presented with secondary IPN within 4 weeks of hospitalization. The clinical data, serum levels of ICAM-1, LPS, and sPD-L1 of the two groups were compared, and the correlation between each peripheral blood index and Ranson score, Acute Physiological and Chronic Health Evaluation Ⅱ (APACHEⅡ) score was analyzed, and the relationship between serum ICAM-1, LPS, and sPD-L1 and secondary IPN of SAP was analyzed. The value of each peripheral blood index to predict IPN secondary to SAP was evaluated. Results In the observation group, Ranson score, APACHEⅡscore, serum levels of ICAM-1, LPS, and sPD-L1 were higher than those in the control group (P<0.05); Pearson correlation analysis showed that the serum levels of ICAM-1, LPS, and sPD-L1 were positively correlated with Ranson score and APACHEⅡ score(P<0.05); Logistic regression analysis showed that serum ICAM-1, LPS, and sPD-L1 were independently correlated with IPN secondary to SAP(P<0.05); Receiver operator characteristic curve analysis showed that the area under the curve(AUC) of serum ICAM-1, LPS, and sPD-L1 to predict IPN secondary to SAP was 0.800, 0.712, and 0.687, respectively. And the AUC of combination of serum ICAM-1, LPS, and sPD-L1 to predict IPN secondary to SAP was 0.876(95% CI: 0.764-0.988), with sensitivity of 76.47% and specificity of 89.29%, which was significantly higher than that of the three index alone. Conclusions Serum levels of ICAM-1, LPS, and sPD-L1 are significantly correlated with the severity of SAP, and the combined detection of the three indexes can be a potential way to predict secondary IPN.

    Nutritional intervention in elderly people with hidden hunger and muscle loss

    Objective To investigate the incidence of malnutrition and hidden hunger of the community elderly, and to investigate the effect of oral nutritional supplement(ONS). Methods A total of 248 elderly people were evaluated by Mini Nutritional Assessment, hidden hunger scale, physical measurement and body composition test. Forty elderly people with hidden hunger and malnutrition were selected and randomly divided into the intervention group and the control group, with 20 cases in each group. The control group received nutrition knowledge education. The intervention group received ONS on the basis of the control group for 12 weeks. The changes of nutritional intake, physical indexes and body composition before and after the intervention were compared between the two groups. Results The incidence rate of malnutrition and hidden hunger in the elderly was 31.5%(78/248). The average intake of fat, protein, zinc, vitamin A, vitamin B1, vitamin B2 and the proportion of high-quality protein in the intervention group were higher than those in the control group(P<0.05). The levels of body weight, body mass index, body fat, plasma total protein and albumin in the intervention group were higher than those before the intervention (P<0.05). Conclusions The incidence rate of malnutrition and hidden hunger is 31.5% in the community elderly. ONS is beneficial to improve the nutritional status, skeletal muscle quality and plasma protein level in the elderly.

    Objective To compare the effects of transnasal humidified rapid-insufflation ventilatory exchange (THRIVE) and mask ventilation on oxygenation and duration of safe apnea after induction of general anesthesia in the elderly patients with tooth less. Methods Fifty elderly patients with tooth less(missing more than 10 teeth) were divided into facemask group (group M) and THRIVE group (group T). In the preoxygenation stage before induction of anesthesia, facemask was used in group M, while THRIVE was used in group T. After the induction of general anesthesia, group M underwent pressure-controlled ventilation, and group T adjusted the flow to 70 L/min. After the muscle relaxant was injected for 4 min, group M stopped the mask ventilation, while group T continued ventilation. When the duration of safe apnea reached 8 min or the level of pulse oxygen saturation (SpO2) dropped to 95%, the experiment was terminated, and the endotracheal intubation was performed immediately with a visual laryngoscope. The cases with duration of safe apnea reaching 8 min, reoxygenation time, and the levels of arterial partial pressure of oxygen(PaO2), arterial partial pressure of carbon dioxide(PaCO2), heart rate(HR), mean arterial pressure(MAP) in different time points were recorded. The ventilation and endotracheal intubation-related complications were recorded and compared between the two groups. Results All the patients in group T had a duration of safe apnea of 8 min, while only 6 patients (24%) in group M had a duration of safe apnea of 8 min (P<0.05). Compared with group M, the reoxygenation time in group T was significantly shorter (P<0.05). The level of PaO2 in the two groups increased after preoxygenation. During simulating apneic tracheal intubation period, the level of PaO2 decreased significantly in group M, while decreased slightly in group T(P<0.05). Compared with the basic level, the level of PaCO2 during apneic period increased signifcantly in both groups, especially in group T (P<0.05). There were no significant differences in the hemodynamic indicators and the incidence rate of the complications related to endotracheal intubation and ventilation between the two groups (P>0.05). Conclusions Compared with facemask, THRIVE can significantly prolong the duration of safe apnea after induction of general anesthesia in the elderly patients with tooth less and reduce the reoxygenation time.

    Effects of different doses of ticagrelor combined with low molecular weight heparin sodium on vascular endothelial function in patients aged≥80 years with unstable angina pectoris

    Objective To study the therapeutic effect of different doses of ticagrelor combined with low molecular weight heparin sodium in the patients aged≥80 years with unstable angina pectoris (UAP). Methods Eighty-six elderly patients aged≥80 years with UAP in our hospital from April 2020 to April 2021 were randomly divided into low-dose group and high-dose group, with forty-three cases in each group. The high-dose group was treated with high-dose ticagrelor (90 mg, twice per day) combined with low molecular weight heparin sodium, and the low-dose group was treated with low-dose ticagrelor (45 mg, twice per day) combined with low molecular weight heparin sodium. The clinical effects of the two groups after 4 weeks of treatment were compared. And the cardiac function indexes [left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF)], inflammatory factors [high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6) ] and endovascular function indexes [endothelin-1 (ET-1), nitric oxide (NO)] before and after 4 weeks of treatment were compared. The adverse cardiac events and bleeding events 6 months after treatment were recorded and compared between the two groups. Results There was no significant difference in the total effective rate between the high-dose group (95.35%) and the low-dose group(90.70%) (P>0.05). After 4 weeks of treatment, the level of LVEDD in the two groups was significantly lower, and the level of LVEF was significantly higher than that before treatment (P<0.05). After 4 weeks of treatment, the levels of hs-CRP, IL-6 and ET-1 in the two groups were significantly lower, and the level of NO was significantly higher than that before treatment(P<0.05), especially in the high-dose group (P<0.05). The incidence rate of adverse cardiac events in the high-dose group was 16.28%, compared with 2.33% in the low-dose group (P<0.05). The incidence rate of bleeding events in the high-dose group was 18.60%, compared with 4.65% in the low-dose group (P<0.05). Conclusions Different doses of ticagrelor combined with low molecular weight heparin sodium shows significant effects in the treatment of UAP. High-dose ticagrelor can inhibit the release of inflammatory factors and protect vascular endothelial function significantly, but low-dose ticagrelor is safer.

    Objective To evaluate the clinical value of ultrasound guidance combined with intracardiac electrocardiogram in the implantment of totally implantable access port (TIAP) in the elderly patients. Methods The clinical data of 204 elderly patients with tumor who received TIAP at our department from January 2018 to December 2021 was retrospectively analyzed. The patients were divided into ultrasound combined with intracardiac electrocardiogram group (group A, n=133) and blind puncture combined with X-ray-based location group (group B,n=71). The successful rate, postoperative complications and unplanned removal rate of TIAP were compared between the two groups. Results The successful rate in group A was 99.2%, compared with 91.5% in group B. Both the incidence rates of short-term and long-term complications in group A were lower than those in group B (P<0.05). The unplanned removal rate of TIAP in group A was also lower than that in group B (P<0.05). Conclusions Ultrasound and intracardiac electrocardiogram is a safe and effective technique, which is an important option for clinical implantation of TIAP in the elderly.

    Effect of limited lined hip prosthesis on femoral neck fracture in elderly patients with hemiplegia

    Objective To investigate the clinical effect of limited lined hip prosthesis in the elderly patients with femoral neck fracture of hemiplegic side. Methods A total of 16 elderly patients suffering from femoral neck fracture of hemiplegic side from 2018 to 2020 in our hospital were enrolled. Pinnacle ES limited lined total hip prosthesis was used for total hip replacement in all patients. The hip function was evaluated by Harris hip score, and the severity of hemiplegic side motor disorder was evaluated by Fugl-Meyer Assessment Scale (FMA), and the hemiplegic lower limb function was evaluated by Brunnstrom staging. The complications during follow-up were recorded. Results The average operation time was 54.7±8.3 min and the average intraoperative bleeding was 198.3±41.6 mL. The average follow-up time was 15.7±3.1 months. The Harris hip score at the last follow-up was significantly higher than that before operation (88.1±8.7 vs 17.4±2.6, P <0.05). The excellent and good rate of hip function was 93.75%. The FMA score at the last follow-up was significantly higher, and the motor function stage of Brunnstrom was significantly higher (P<0.001) than those before operation. During the follow-up, osteolysis, prosthesis loosening, sinking and periprosthetic fractures were not found. Popliteal vein thrombosis occurred in one patient (6.25%). Conclusions The treatment of limited lined hip prosthesis for femoral neck fracture in the elderly with hemiplegia can effectively improve the function of hip joint and hemiplegic side limb, and can reduce the postoperative dislocation rate of prosthesis.

    Analysis of related factors of nucleic acid shedding in elderly patients infected with Omicron variant of SARS-CoV-2

    Objective To analyze the nucleic acid shedding time of Omicron variant of novel coronavirus in the elderly patients with non-severe infection, and to explore the related factors affecting the nucleic acid shedding time. Methods A total of 104 elderly patients with non-severe COVID-19 were divided into early negative group (<10 days) and late negative group (≥ 10 d) by the nucleic acid shedding time. The population information, vaccination, previous diseases, blood biochemical and inflammatory indicators, nucleic acid ORF1ab gene and N gene Ct values were collected and compared between the two groups. The Spearman rank correlation and multiple linear regression were conducted to explore the influencing factors of the nucleic acid shedding time. Results The mean time of nucleic acid shedding of Omicron variant in the early negative group was 7.26±1.54 d, compared with 12.96±2.44 d in the late negative group. There were significant differences in age, the ratio of chronic heart failure, chronic pulmonary disease and booster vaccination for COVID-19 and the first nucleic acid Ct value between the two groups (P<0.05). Spearman correlation analysis showed that the nucleic acid shedding time of Omicron was positively correlated with age, chronic heart failure and serum level of procalcitonin, but negatively correlated with the vaccination booster and the first tested nucleic acid Ct value. Multiple linear regression analysis showed that age, vaccination booster and the first tested nucleic acid Ct value were the independent influencing factors of the nucleic acid shedding time. Conclusions Age, vaccination booster for COVID-19 and the first tested nucleic acid Ct value were the independent influencing factors of nucleic acid shedding in the elderly non-severe patients infected by SARS-CoV-2 Omicron. Vaccination booster for COVID-19 in the elderly vulnerable groups can shorten the time of nucleic acid shedding.

    Objective To observe the clinical curative effects between improved Zhao's thunder fire moxibustion therapy and self-made Chinese medicine package external application therapy in the treatment of elderly patients with temporomandibular joint disorders(TMD). Methods A total of 58 patients with TMD were randomly divided into the treatment group and the control group with 29 cases in each group. The treatment group was treated with sparrow-pecking moxibustion in the tender area and the Xiaguan acupoint while being locally massaged in the joint area with the medicinal stick. The control group was treated with the locally heated external application therapy using traditional Chinese medicine package. Results The pain VAS score of the treatment group was improved significantly compared with the control group. The mouth opening degree of the treatment group was significantly improved after treatment. The total effect of the treatment group was higher than that of the control group(P<0.05). Conclusions The thunder fire moxibustion therapy is significantly superior to the external application of traditional Chinese medicine in the treatment of elderly patients with temporomandibular joint disorders syndrome.

    Investigation and research on the status of frailty and management cognition of elderly patients with lung cancer

    Objective To investigate the status of frailty and management cognition in the elderly patients with lung cancer,so as to provide references for management of frailty in the elderly to the oncology medical staff. Methods A total of 167 elderly patients with lung cancer were surveyed with the general information questionnaire and Groningen Frailty Indicator Scale (GFI), and 8 cases of them were interviewed in a semi-structured way. Results The incidence rate of frailty among 167 elderly patients with lung cancer was 70.1%.The total score of GFI was 4.69±2.51. The education levels, residence, and income were the influencing factors of frailty score (P<0.05). Four themes and eight sub-themes were extracted from the semi-structured interview,including lack of knowledge (incomplete understanding of the concept of frailty in old age, different opinions on the causes of frailty in old age), insufficient self-management (low confidence, loose consciousness), accumulation of negative emotions (inner fear, helpless feeling), insufficient resource protection (time restrictions, longing for support). Conclusions Medical professionals in oncology should pay attention to the frailty screening of elderly lung cancer patients with low income, low education level or living at rural areas, and they also need to strengthen frailty knowledge education, improve self-management capabilities and other ways to solve the problem of frailty management in the elderly.

    Objective To develop a questionnaire about the knowledge, attitude, practice (KAP)and training needs of clinical nurses on palliative care, and to test its reliability and validity. Methods Through referring to relevant literature and teaching materials, expert interviews, nurse communication and two rounds of Delphi expert consultation, the first version of clinical nurses' KAP and training needs questionnaire on palliative care was formed. A total of 204 clinical nurses were selected for survey by convenient sampling method. The items were analyzed and screened by the method of division analysis, correlation coefficient and factor analysis. The reliability of the questionnaire was tested by internal consistency analysis, split-half reliability and retest reliability. The validity of the questionnaire was evaluated by content validity and structure validity, and the final questionnaire was formed. Results The questionnaire consisted of four dimensions, with four items in nursing knowledge dimension, six items in attitude dimension, nine items in behavior dimension and ten items in training needs dimension. The Cronbach's α coefficient of total questionnaire was 0.938, and its split-half reliability and test-retest reliability of the questionnaire was 0.703 and 0.868, respectively. The content validity of the questionnaire was 0.919. Four common factors were extracted by exploratory factor analysis, and the cumulative variance contribution rate was 72.335%. Conclusions The questionnaire has good reliability and validity, and can be used to evaluate the level of KAP of hospice care and training needs of the nurses.
