Abstract: Objective To explore the relationship of fibrosis 4 score(FIB-4) with Gensini score in the elderly patients with acute coronary syndrome(ACS). Methods A total of 146 elderly patients with ACS were enrolled in the study, and were divided into the low Gensini group(≤37) and high Gensini group(>37) according to Gensini score. The general data and laboratory indexes were compared between the two groups. Multivariable logistic regression analysis was performed to identify the influencing factors of Gensini score. Receiver operating characteristic curve analysis was used to explore the predictive value of FIB-4 for the severity of coronary artery lesions in the patients with ACS. Results There were statistically significant differences in body mass index(BMI), white blood cell count, neutrophils, aspartate aminotransferase(AST), alanine-aminotransferase(ALT), total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), uric acid(UA), Gensini score, triglyceride-glucose index(TyG), FIB-4 and neutrophil-to-lymphocyte ratio(NLR) between the two groups. Multivariate logistic regression analysis showed that UA, TyG, FIB-4 and NLR were independent risk factors for the increase of Gensini score. The area under the curve of FIB-4 in predicting the increase of Gensini score was 0.79, which was higher than that of NLR, TyG and UA alone. Conclusions FIB-4 is positively correlated with Gensini score, and it can be used as economic and easily measured laboratory indicator to predict the severity of ACS in elderly patients.
